Experts state that Parkinson's disease, defined as "a progressive disease characterized by problems such as slowness in movements, tremor, and gait disturbance and falls," starts insidiously and unilaterally, making it difficult to detect. Experts note that when a doctor is consulted, it is found that the disease had started 1-2 years earlier, and they emphasize that fasting is medically inconvenient in Parkinson's disease due to medication use. Experts point out that fasting can lead to a state of immobility, referred to as 'freezing,' and hospitalization in the patient.
To raise social awareness and ensure consciousness, April 11 is commemorated every year as World Parkinson's Disease Day.
Üsküdar Üniversitesi NPİSTANBUL Hastanesi Neurology Specialist Dr. Celal Şalçini, in his statement within the scope of World Parkinson's Disease Day, addressed the types, symptoms, diagnosis, and treatment processes of the disease, as well as the disadvantages of fasting during Ramadan, and shared important recommendations.
**1-2 years have passed when the disease is noticed**
Neurology Specialist Dr. Celal Şalçini stated that Parkinson's disease is a very old disease named after the person who discovered it, and said, "It is a progressive disease usually characterized by problems such as slowness in movements, tremor, gait disturbance, and falls. It starts insidiously and unilaterally, making it difficult to detect. By the time the patient consults a doctor, the disease has already started 1-2 years prior. Parkinson's begins in a person as a result of a deficiency in a structure called a neurotransmitter in the brain or damage occurring along this pathway."
**There are 2 types of Classical Parkinson's**
Neurology Specialist Dr. Celal Şalçini stated that there are 2 different types of Parkinson's: akinetic-rigid and tremor-dominant, and said, "They can be defined as Parkinson's progressing with slowness and Parkinson's with active tremor. Sometimes these two types of Parkinson's can start simultaneously, but this is rarely seen. Regardless of the type, both tremor and slowness begin unilaterally. After some time, it progresses to the other side and becomes bilateral. In Parkinson's that progresses with slowness, it is somewhat more possible to get a response to treatment. In Parkinson's with tremor, stopping the tremor is somewhat more difficult and requires higher doses of medication. Of course, in advanced stages, alongside tremor, disorders such as forgetfulness, certain specific problems, and brain thinning can occur. These are classical Parkinson's disease."
**Beware of the Poker Face expression…**
Neurology Specialist Dr. Celal Şalçini continued his words by stating that there is Parkinson's disease with additional syndromes called Parkinson Plus, in addition to akinetic-rigid and tremor-dominant types:
"The general characteristic of these disorders is that they are not as 'face-friendly' as Parkinson's. Their treatments are difficult, they are less responsive to medications, their course is more severe, and they progress faster. They do not continue solely with Parkinson's symptoms. In addition to Parkinson's symptoms, autonomic system dysfunction, restricted upward gaze, problems with hand use, spasms, imbalance, cerebellar atrophy, and early-stage cortical atrophy in the brain may be observed. When we see patients with this Parkinson's disorder, we encounter some symptoms. First of all, a dull expression dominates their faces. Their use of facial expressions has greatly decreased. It is referred to as 'poker face expression' in books. A decrease in the patient's blinking rate is observed. There are lesions and crusting on the facial skin. They usually walk with small steps, leaning forward. They have imbalance and a risk of falling."
**Medication is used to confirm the diagnosis**
Neurology Specialist Dr. Celal Şalçini stated that examination would be sufficient to diagnose Parkinson's, and said, "At this point, a thorough examination is important. Obtaining help from imaging devices and equally from blood tests is of great importance. We want to rule out all of these. Because Parkinson's can also cause a sudden clot in the brain. It can also lead to the formation of some substances like copper accumulation. Therefore, patient images will be needed for differential diagnosis. In Parkinson's disease, medication is started to confirm the diagnosis. If the medication shows benefit, then this disorder is definitely Parkinson's. If the medication does not show benefit, then the disorder is Parkinson Plus or a different disease. This situation is called 'test thérapeutique,' a French term. This means the physician can sometimes go from medication to diagnosis. According to recent research, starting medication early in the diagnosis of Parkinson's disease does not affect the patient's life span. We diagnose the patient. Early diagnosis is important, of course, because the patient should know what kind of disorder they have. However, even with an early diagnosis, we delay medication treatment."
**Quality of life is increased with medication**
Neurology Specialist Dr. Celal Şalçini stated that while Parkinson's treatment is not possible, the medications given improve the patient's quality of life, and said, "Medications at least prevent the patient's tremor and slowness. Thus, the patient can continue their life in a normal course for a long time. The strategy followed here is this: When medication treatment is started for a patient, it begins with the lowest possible dose, and the dose is increased according to the patient's needs. Because these medications have side effects. These side effects are dose-dependent and time-dependent. In other words, the higher the dose and the longer a patient uses high-dose medication, the higher the potential for side effects."
**Fasting is inconvenient for Parkinson's patients…**
Neurology Specialist Dr. Celal Şalçini pointed out that in Parkinson's disease, medications may need to be given three or more times a day, and sometimes even every 3-4 hours, and said, "Especially in these situations, fasting is medically inconvenient. The sudden discontinuation or reduction of medication causes further slowing of the patient's movements or a significant increase in tremor. This slowness can sometimes affect swallowing and lead to the patient becoming immobile, which we call 'freezing' in medical terms, requiring hospitalization."
**Genetic predisposition depends on many factors**
Neurology Specialist Dr. Celal Şalçini stated that a very small portion of Parkinson's disease is genetically inherited, and said, "This familial Parkinson's develops due to family members and starts at a young age. It is detected with genetic tests, which are also available in Turkey. There is Parkinson's disease that starts around the age of 45. Its prognosis is naturally poor because it is genetic. Medications respond a bit less, but fortunately, it is rare. On the other hand, there is also a genetic predisposition. It's not definitive, of course; many factors need to be present together. Not only for Parkinson's but also for diseases involving brain cell death like Alzheimer's, a genetic infrastructure exists. However, genetic predisposition alone is not a sufficient factor. Furthermore, there are many hypotheses regarding a person's lifestyle triggering Parkinson's, but none are definitive."
**Main symptoms are slowness and tremor**
Reminding that slowness and tremor are the main symptoms of Parkinson's, Neurology Specialist Dr. Celal Şalçini said, "Everyone with hand tremors must come for an examination. Along with this, a condition called asocial movement is observed in the arm and leg, where one limb can be moved but the other cannot. Mental slowness is also a factor in this disease. Tremor can have many causes. It does not necessarily have to be due to Parkinson's. The examination is performed with the help of imaging devices. An EMG device can be used to be sure. Then a diagnosis is made, and treatment is started."
**Patient, doctor, and caregiver must be in communication**
Emphasizing that cooperation between the patient, caregiver, and doctor is very important, Neurology Specialist Dr. Celal Şalçini said, "Because this is an incurable disease, it is more focused on increasing the comfort of the patient and their caregiver. The adaptation process and the doctor's accessibility to the patient are very important here. Since it is a progressive disease, the patient needs to consult the doctor frequently. They need to be good observers. We usually ask the patient, 'Did the medication we gave you 'open you up' (i.e., improve your symptoms)?' In other words, the medication we give should actually start working on the patient within 30-40 minutes. The dosage is determined according to the patient's reaction to the medication."
