Dr. K. Ali Rahimi from Üsküdar Üniversitesi NPİSTANBUL Hastanesi touched upon the causes of ear pain caused by pressure during airplane travel and offered important advice to prevent potential problems. ## İçerik During airplane travel, especially when descending, very severe ear blockage, ear ringing, and even sometimes very severe bleeding can occur due to changes in pressure speed. ENT Specialist Op. Dr. K. Ali Rahimi states that it is natural to feel pressure in the ear canal during takeoff, landing, and flight if there are no problems in the outer ear canal, but if there is a blocked nose during the journey, negative pressure forms in the middle ear, causing severe pain. ENT Specialist Op. Dr. K. Ali Rahimi recommends chewing gum or candy before the trip to prevent ear blockage and advises consulting a specialist if there is a flu infection, as flying with a blocked nose can cause pressure and pain in the ear. Ali Rahimi said, “The amount of air in our middle ear passes into the middle ear only through the nose and the Eustachian tube, and the pressure inside the middle ear can be adjusted via the Eustachian tube. The middle ear does not have an opening to the outside world. If our outer ear canal is normal, it can be exposed to various pressures when the plane lands, takes off, and is in cruising altitude. In this case, the pressure of the outer ear canal is adjusted according to the cabin pressure. Especially when the plane begins its descent, very severe ear blockage, ear ringing, and sometimes even very severe bleeding occur due to changes in pressure speed. Although all of these appear to be related to ear blockage, they are a situation related to nasal congestion.” **Severe pain occurs when pressure cannot be adjusted** Stating that when nasal congestion and its component, the Eustachian tube, fail to adjust the pressure in the middle ear, the eardrum experiences a suction effect inwards due to negative pressure, Op. Dr. K. Ali Rahimi said, “This suction of negative pressure causes very severe pain. If this pain intensifies, it draws fluid from small and capillary vessels into the middle ear, and if the pain increases further, this time it draws blood into the middle ear, thus causing much more severe pain in the ear.” **Valsalva maneuver must be performed to prevent pain** Stating that it is possible to activate the Eustachian tube and relieve negative pressure by sending more air from the Eustachian tube to the middle ear, Op. Dr. K. Ali Rahimi said, “Thus, it is necessary to perform the movement known as the Valsalva maneuver, which involves trying to exhale with moderate force while the airway is closed. First, we block our nose with our hand, and then we forcefully push air through our cheeks towards the middle ear. This Valsalva maneuver should be repeated several times.” **Chewing gum can prevent ear blockage** Recommending that individuals who experience ear blockage problems before an airplane trip chew a small piece of candy or gum throughout the journey, Op. Dr. K. Ali Rahimi said, “Thus, with jaw movements and swallowing, the Eustachian tube moves, air opens up and passes into the middle ear. In this way, negative pressure is prevented. Another way is to move the jaw forward and backward.” **Flying with a blocked nose causes pain** Op. Dr. K. Ali Rahimi said, 'If fluid accumulation occurs in the middle ear, this condition continues for 2-3 weeks after the airplane journey, and it is necessary to see a specialist immediately.' He concluded his words as follows: After an examination, the specialist doctor may prescribe a nasal spray. In some individuals, this condition experienced during airplane travel can lead to permanent problems. This is a very rare situation. Generally, this situation is observed in people who travel frequently by plane. For these individuals, tubes are inserted into the eardrum to try and adjust the air pressure on both sides not through the Eustachian tube, but through the eardrum. If there is a flu infection before airplane travel, a specialist should be consulted.
