## Özet

**Psychiatrist Prof. Dr. Nevzat Tarhan stated that prioritizing external appearance or excessively preoccupation with it, magnifying something perceived as a flaw in one's body, is known in psychiatry as ‘Body Dysmorphic Disorder,’ adding, “In everyday language, we can also describe this as the fear of being ugly, or ugliness phobia.”**

**Prof. Dr. Nevzat Tarhan said, “What truly makes a person beautiful is their charm; people are greeted by their physical appearance but are remembered by their personalities… Therefore, physical appearance should not be the priority. These individuals cannot make peace with themselves.”**

Üsküdar Üniversitesi Founding Rector, Psychiatrist Prof. Dr. Nevzat Tarhan evaluated the issue of obsession with external appearance.

## **“It has a wide spectrum ranging from a simple obsession with external appearance to a disease”**

Prof. Dr. Tarhan noted that prioritizing external appearance or excessively preoccupation with it, magnifying something perceived as a flaw in one's body, is known in psychiatry as ‘Body Dysmorphic Disorder,’ adding, “In everyday language, we can also describe this as the fear of being ugly, or ugliness phobia. For example, there's a fear of being overweight. A person weighing 29 kilograms might feel like they weigh 150 kilograms. This person's entire daily life begins to be disrupted. Their sole topic of conversation, their sole area of interest becomes that. They research it online, and after a while, it turns into a delusion. After a certain period, it can transform into what we call somatic delusion. It has a wide spectrum, ranging from a simple obsession with external appearance to a disease. In recent years, we've seen a significant increase in this, especially among young people, where physical appearance has become very important.”

## **The perception of ‘If you are beautiful, you are valuable; if you are not beautiful, you are worthless’ is increasing among us too…**

Prof. Dr. Tarhan stated that as a natural characteristic of youth, physical appearance comes to the forefront with adolescence, adding, “If a child grows up with a perception in the family like ‘if you are beautiful, you are valuable; if you are not beautiful, you are worthless,’ and if such a value judgment exists, then this becomes a learned behavior. This is very common in Western culture, and it is gradually increasing among us too. We have started to encounter more cases related to this among young people. Especially, even the healthiest person who sees a fashion magazine feels ugly. It is processed that way. They have a famous saying: ‘’There are no ugly women, only unkempt women.’ This has led to the formation of a serious industry, also using cultural propaganda.”

## **“Obsession with external appearance has started to be seen significantly in men too”**

Prof. Dr. Tarhan, stating that obsession with external appearance has started to be seen significantly in men, also pointed to the level Turkey has reached in hair transplantation.

Prof. Dr. Tarhan, stating, “People's priorities have changed a lot. An era focused on external appearance and pleasure is coming,” explained that psychological resilience was discussed at the World Economic Forum in Davos because psychological resilience also affects the economy.

Prof. Dr. Tarhan, pointing out that people's spending and purchasing behaviors have changed, said, “People are becoming more expedient, avoiding difficult tasks. This has also negatively affected the supply chain. In this economy, people are not applying for difficult jobs. The pandemic also has an impact on this. Of course, not everyone is affected by this.”

## **The impact of the constant pursuit of beauty and handsomeness on personality disorders…**

Prof. Dr. Tarhan, also evaluating the impact of the constant pursuit of beauty and handsomeness on personality disorders, noted the following:

“This effort is also present in the symptoms of personality disorders. We say of such individuals, ‘they made their narcissistic investment in their physical appearance.’ What do we understand by a person's narcissistic investment? It's an investment of love... Whatever a person invests their love in, they focus on that point. The most narcissistic being known is a child. They express their love themselves, imagine themselves at the center of the world, and think everyone exists for them. As they grow, they start saying ‘me and others’ and learn about other objects of love. As they grow, they invest their love in their country, homeland, humanity, nature, and the creator, all of them. This varies from person to person.”

Prof. Dr. Tarhan stated that individuals who obsessively invest their love in their external appearance react as if doomsday has arrived if even a small pimple appears on their body, adding that these individuals frequently visit aesthetic specialists.

## **“When a person sees aging as a threat, anxiety increases”**

Prof. Dr. Tarhan also emphasized the effort to avoid aging and explained the following:

“Fear of aging is one of the most common things encountered in this era. For a person to be happy, they need to be accepting. It used to be called anti-aging; now in conferences, it's called healthy aging. When a person sees aging as a threat, anxiety increases. Seeing aging as a threat is like seeing rainfall as a threat. It's a natural condition. The only undeniable truth of life is the truth of death. When you don't accept this, when you don't accept aging, it leads the person to anxiety and fear. Living with fear also makes one a candidate for depression. The person tries to relieve themselves by indulging more in entertainment. Or they reconstruct their philosophy of life. This leads to wisdom. A little fear of aging is expected.”

## **“Humans can live up to 140 years depending on lifestyle and genetics”    **

Prof. Dr. Nevzat Tarhan emphasized that being uneasy about aging is normal and highlighted the importance of accepting aging, stating, “They ask a 92-year-old lady, who is very happy, ‘What do you owe this happiness to?’ She says, ‘I think about my functioning organs, not my non-functioning ones.’ With the same perspective, if a person cannot change the flow, they will change their perspective… In this situation, old age is an unchangeable reality.”

Prof. Dr. Tarhan also suggested that individuals should accept aging and focus on how to age healthily, saying, “A person grows with what they eat up to a certain age, and lives with what they don't eat after a certain age. For this reason, currently, 60-70% of some diseases stem from wrong nutrition and wrong lifestyle. Humans can live up to 140 years depending on lifestyle and genetics.”

## **What is body image disorder?**

Prof. Dr. Nevzat Tarhan also touched upon body image disorder, continuing as follows:

“There is a test for this. When you do the test, various body types are shown to the patient: thin, medium, overweight. The patient is asked in which of these they find their own body. We ask them to indicate which part of their body they don't like. The person marks the parts they dislike on their own body in those visuals. We treat them. After treatment, we perform the test again. This time, regarding the parts of their body they disliked, they say, ‘I obsessed too much, I worried for nothing.’ These individuals have a physical fear of being ugly. 

It is more common in women than men. More common among young people, but among men, more common in older, single, and solitary men. This condition becomes a disease as a result of a person's disproportionate interest in their body, constantly obsessing disproportionately over a physical flaw. In such cases, the person expends all their resources on this. They take out loans, get surgeries, or go to beauty centers. When we investigate, most of them have experienced adverse life events in their childhood. We frequently see childhood traumas in these patients.”

## **“Individuals whose priority is physical appearance cannot make peace with themselves”**

Prof. Dr. Nevzat Tarhan stated that what makes a woman beautiful is not her physical appearance but her charm, adding, “What truly makes a person beautiful is their charm; people are greeted by their physical appearance but are remembered by their personalities… Therefore, physical appearance should not be the priority. These individuals cannot make peace with themselves. Some of these individuals believe they are excessively overweight. Some, like transsexuals for example, feel trapped in the wrong body. They try to change their body because they believe they are in the wrong body.”

## **“When the brain's infrastructure improves, therapy also works”**

Prof. Dr. Tarhan stated that individuals with obsession with external appearance can be treated, explaining, “The brain's network is disrupted in these individuals. The brain areas related to their physical appearance become wide like a highway… Because so much investment is made there, those areas develop much more in the brain. Therefore, these individuals are hospitalized, treated, and the body part the brain is obsessed with is brought to mind, and provocation is performed. With visual provocation, magnetic stimulation is given to that area of the brain at that moment. If the hypersensitivity and receptor sensitivity in that area of the person's brain are high, they are brought down. When the brain's infrastructure improves, therapy also works.”
