## Özet

Experts recommending routine dental examinations before school say this can help identify children's unnoticed oral and dental problems at an early stage.

## İçerik

Asst. Prof. Buse Yılmaz Şen from the Department of Pediatric Dentistry, noting that dental caries do not always cause pain, said, 'A child having no complaints does not mean there are no problems with their oral and dental health. When pain emerges, the problem may have reached a more advanced stage.' Asst. Prof. Şen stated that regular dental visits help children get used to the clinical environment and reduce dental fear, recommending that families make oral and dental health checks an indispensable part of the school preparation list.

Asst. Prof. Buse Yılmaz Şen from the Department of Pediatric Dentistry at Üsküdar University Dental Hospital made statements about the importance of children undergoing oral and dental health checks before school.

**Dental check-ups should not be neglected before school starts! **

Asst. Prof. Buse Yılmaz Şen stated that as the new academic year approaches, children's school needs such as stationery and clothes are completed, but often oral and dental health checks are left out of the preparation list. She said, 'However, a routine dental examination carried out before school is important for identifying undetected cavities, gum problems, eruption disorders, and oral habits at an early stage.'

Asst. Prof. Şen pointed out that oral and dental health in children is closely related to general health, nutrition, sleep, and school performance. She stated, 'Families should not neglect dental check-ups before school starts. Dental caries may not always cause pain in its initial stages. A child having no complaints does not mean there are no problems with their oral and dental health. When pain emerges, the problem may have reached a more advanced stage. Therefore, a pre-school check-up allows for the evaluation of potential problems before the child experiences pain and before their school routine is affected. Oral and dental problems should not be considered limited only to the mouth. Pain, difficulty chewing, and sleeplessness can directly affect a child's daily life. The need for emergency dental treatment during the school period can also lead to the child missing classes and disruptions in their school routine. The primary goal of a preventive approach is to take measures before the problem escalates.'

**Deciduous teeth should not be neglected with the thought of 'they will fall out anyway'!**

Asst. Prof. Şen noted that there is a misconception in society that deciduous teeth do not need treatment because they are temporary. She said, 'Deciduous teeth play important roles in a child's healthy nutrition, proper chewing, and speech development.'
Asst. Prof. Şen reminded that deciduous teeth hold space for permanent teeth in the mouth and guide their eruption into the correct position. She added, 'A prematurely lost deciduous tooth can cause adjacent teeth to shift into the empty space, leading to crowding later on. Therefore, a cavity in a deciduous tooth should not be postponed with the thought of 'it will fall out anyway'; it should be evaluated by a pediatric dentist.'

**The first dental visit should not start with an emergency!**

Asst. Prof. Şen emphasized that a child's first encounter with a dentist during a painful and emergency treatment can increase anxiety, adding that routine check-ups also help the child get used to the clinical environment.

Şen said, 'The relationship of trust is very valuable in pediatric dentistry.' She continued, 'A brief and positive introductory examination when there are no complaints allows the child to recognize the dentist not as someone to be feared, but as someone who protects their health. Through regular visits, the child not only becomes familiar with the environment but also learns that oral care is a natural health habit that should be maintained throughout life.'

**What is checked during a pre-school examination?**

Asst. Prof. Şen noted that a routine dental examination is not limited only to checking for cavities. She stated, 'During the examination, according to the child's age and needs; whether there are incipient or advanced cavities, the condition of the gums and intraoral tissues, the eruption process of deciduous and permanent teeth, chewing and occlusion relationships, the risk of premature deciduous tooth loss or crowding, habits such as thumb sucking, mouth breathing, lip or pencil biting, tooth brushing technique and oral hygiene habits, and the need for preventive applications based on the child's caries risk are taken into consideration.'

Asst. Prof. Şen noted that every child's oral structure, eating habits, and caries risk are different, and added:
'The check-up interval should be determined by the pediatric dentist's individual assessment rather than a standard calendar. Depending on the child's age and caries risk, preventive procedures such as fluoride applications, fissure sealants, or oral hygiene education may be planned as a result of the examination. The aim is not only to treat existing problems but also to reduce the risk of new problems developing.'

**The responsibility for tooth brushing should not be entirely left to the child at a young age!**

Asst. Prof. Buse Yılmaz Şen stated that the manual dexterity of pre-school children may not always be sufficient to effectively clean all tooth surfaces. She said, 'The child should be allowed to brush their own teeth, but the final check and necessary completion should be done by a parent.'

Asst. Prof. Şen stated that brushing should not be presented as a chore or punishment. She added, 'It should be made a natural part of the daily routine, morning and evening. The selection of an age-appropriate toothbrush and fluoride toothpaste should be made according to the pediatric dentist's recommendation. The amount of toothpaste used should be adjusted according to the child's age.'

**Avoid phrases that increase dental fear!**

Asst. Prof. Şen stated that the language used by parents before the examination affects the child's approach. She warned, 'While phrases like 'Don't be afraid, it won't hurt, no needles will be used' may be well-intentioned, they can create associations of pain and fear in the child's mind.'

Asst. Prof. Şen suggested, 'Simple and positive phrases such as 'we will count your teeth, we will help your teeth grow healthy' can be used for the child. It is also important not to recount negative past dental experiences in front of the child. Explaining the examination briefly, honestly, and in an age-appropriate manner, rather than making it a surprise event, supports the child's trust.'

**Oral and dental health check-up should be part of the school preparation list!**

Asst. Prof. Şen underlined that a brief check-up before school can prevent more comprehensive problems that might arise during the term, and concluded her words as follows:

'Healthy teeth are important for comfortable chewing, quality sleep, clear speech, and a confident smile. We recommend that families consider oral and dental health as an integral part of the school preparation process. The goal for children is not to intervene when pain begins, but to maintain oral health with regular check-ups and correct habits.'
