Ali Rahimi from Üsküdar Üniversitesi NPİSTANBUL Hastanesi stated that the secretion known as earwax is not actually a dirty tissue, but rather a fatty tissue secreted by the ear. ## İçerik **An expert must be consulted for ear cleaning** ENT Specialist Op. Dr. K. Ali Rahimi stated that the secretion commonly thought to be 'earwax' in society is not actually a dirty tissue, but a fatty tissue secreted by the ear, and that this fatty tissue especially protects the ear from water. Rahimi, noting that cleaning the outer ear canal with an ear swab can lead to significant problems, said, “This fatty tissue, which should come out, is pushed inwards when an ear swab is used, and the ear canal becomes blocked.” Rahimi emphasized that an expert must be consulted for ear cleaning. Dr. K. Ali Rahimi stated that this fatty tissue is located under the skin and is very soft, adding, “Since sound is not transmitted from soft areas, the skin is attached to the outer ear canal firmly and without fat. However, fat-free skin cannot survive. Fat secretes into the canal of the outer ear canal. It keeps that ear constantly oily. Thus, water cannot enter the ear; water repels oil. People’s complaints related to earwax are usually in the form of ear blockage, reduced hearing, tinnitus, dizziness, dry cough, ear pain, discharge, and itching. These complaints can occur in one or both ears.” **Ear swabs should not be used** Op. Dr. K. Ali Rahimi noted that some people produce more earwax, stating, “Dust and dirt entering the ear during the day stick to the secretion called cerumen, which is present in the outer ear canal, and dryness occurs. When the secretion dries, earwax is pushed from the outer ear towards the auricle. Earwax is more common in people working in dusty and dirty environments. The problem starts with cleaning the outer ear canal with an ear swab. If an ear swab is used there, this fatty tissue turns black or brown because it comes into contact with air. In some people, this fatty tissue is secreted more abundantly. This fatty tissue, which should exit, is pushed inwards due to the use of an ear swab, and the ear canal becomes blocked.” Op. Dr. K. Ali Rahimi noted that the earwax produced in the ear has an acidic structure, warning, “Therefore, it acts as the body’s natural defense mechanism against infections that occur here. However, when constantly exposed to conditions such as moisture and scratching, earwax is consumed, and the ears become susceptible to infection.” **It can lead to hearing loss** Op. Dr. K. Ali Rahimi stated that when earwax dries due to the ear’s structure, it is pushed towards the auricle, adding, “However, sometimes earwax can accumulate in the outer ear canal. Therefore, problems such as hearing loss can occur. Typically, around 25 percent hearing loss is observed. However, normal hearing occurs in the ear.” **An expert should be consulted for ear cleaning** Op. Dr. K. Ali Rahimi stated that an expert must be consulted for earwax removal, saying, “The specialist doctor cleans it by pulling it out with a tool called a curette or by suctioning the earwax out with a negative-pressure aspirator. Some earwax is very hard and does not come to the tip of the aspirator. If it does not come to the tip of the aspirator, it is first softened using oily tissue medications and then easily cleaned with the aspirator.” **Constantly cleaning the ear increases the risk of infection** Op. Dr. K. Ali Rahimi emphasized that earwax removal in children should only be done by the relevant specialist doctor, warning, “If the ear’s fatty tissue is not very dense, the ear is not blocked. Unblocked ears in children, just like in adults, should not be cleaned. Furthermore, cleaning the ear with an ear swab is harmful. Constantly cleaning the ear, constantly poking it with something, both increases the risk of infection and causes dirt accumulated on the outside to be pushed inwards. Especially do not use ear cleaning swabs after bathing, because ear swabs push the accumulated debris on the outer wall of the ear further inside, blocking the ear more, and removal is very difficult, especially in children.” **Do not poke your ears!** Op. Dr. K. Ali Rahimi noted that if the ear’s fatty tissue is not very dense, the ear will not be blocked, concluding his words by advising, “In that case, you do not need to clean your ear. If you remove the skin’s fatty tissue, the skin will dry out, making the ear susceptible to both infections and itching.
