## Özet

Schizophrenia treatment was discussed at the ECNP Congress! The 39th Congress of the European College of Neuropsychopharmacology (ECNP) was held in Munich, Germany.

## İçerik

Psychiatrist Prof. Nesrin Dilbaz, who gave a speech at the congress, drew attention to the importance of planning continuous treatment from the first episode of schizophrenia. Prof. Dilbaz said, “Difficulties in continuing treatment cannot be explained solely by the patient's behavior. Side effects, forgetfulness, difficulties in disease awareness, stigma anxiety, and problems with access to healthcare services can affect treatment continuity.” In her speech, Prof. Dilbaz emphasized an approach that supports the individual's return to education, work, and social life, in addition to preventing the recurrence of the disease.

Psychiatrist Prof. Nesrin Dilbaz from Üsküdar University NPİSTANBUL Hastanesi delivered a speech at the 39th Congress of the European College of Neuropsychopharmacology (ECNP) on early intervention in schizophrenia treatment, personalized treatment selection, and participation in daily life.

Prof. Dilbaz drew attention to the continuity of treatment to preserve the recovery achieved after the first psychotic episode.

Prof. Dilbaz stated that first-episode treatment offers an important opportunity to prevent future disease periods, and conveyed the message that the treatment plan should also cover the period after acute symptoms subside.

**Continuity of treatment is important for preserving recovery!**

Prof. Dilbaz reported that research shows a significant response to treatment can be achieved in first-episode schizophrenia, but added, “However, interrupting or disrupting treatment after symptoms subside can increase the risk of disease relapse.”

Prof. Dilbaz pointed out that a new episode can lead to increased distress for the individual, hospitalization, and interruption of education or working life. “Therefore, preserving the recovery achieved in the early stages is among the long-term treatment goals. Difficulties in continuing treatment cannot be explained solely by the patient's behavior. Side effects, forgetfulness, difficulties in disease awareness, stigma anxiety, and problems with access to healthcare services can affect treatment continuity,” she stated.

**Long-acting treatments can be considered early!**

Prof. Dilbaz also touched upon long-acting antipsychotic treatments administered via injection at specific intervals, saying, “These options can support treatment continuity in some patients who have difficulty maintaining daily medication use.”

In her speech, Prof. Dilbaz referred to a study involving 83 patients who had recently experienced their first schizophrenia episode, stating the following:

“In 12-month follow-up, disease symptom exacerbation or recurrence was reported as 5 percent in the long-acting injection group and 33 percent in the group using the same medication orally. This result cannot be directly generalized to all patients or all injection treatments, as it belongs to a specific medication and patient group.

Instead of introducing long-acting treatments after the disease recurs, they should be discussed as options early on in suitable patients. In treatment selection, the patient's expectations, previous medication experiences, and possible side effects must be evaluated together.”

**Treatment should be determined according to the patient's preferences and attitudes!**

Prof. Dilbaz underlined that long-acting injections may not be equally suitable for every patient, stating, “The drug's effect and tolerability, administration interval, and the individual's life circumstances are important in the decision-making process. Clinical guidelines also recommend considering the patient's preferences and attitudes when evaluating these treatments.”

Prof. Dilbaz also stated that longer administration intervals can reduce the daily treatment burden for suitable patients, but added that extending the injection interval does not mean reducing regular check-ups and psychosocial support.

**The goal is to support recovery with a sustainable care plan!**

Emphasizing that first-episode care should be conducted within a comprehensive support program, Prof. Nesrin Dilbaz said, “In addition to medication, psychoeducation, collaboration with family, psychological support, and planning for return to education or work are components of this approach.”

Prof. Dilbaz concluded her remarks by saying, “In evaluating treatment outcomes, the individual's ability to maintain friendships, live independently, and progress towards their own goals is also important. Recovery should be supported from the first episode with a personalized and sustainable care plan.”

At the congress, which featured leading experts from around the world, Prof. Alkomiet Hasan from Germany chaired the session, while Dr. Vassilis Martiadis from Italy also participated as a speaker.
