## Özet

**Psychiatrist Prof. Dr. Nevzat Tarhan noted that long-term nicotine use impairs the brain's learning memory, stating, “Nicotism is not harmless. When you suddenly quit nicotism, you experience severe headaches, restlessness, and irritability. They become dazed and suffer from inattention.”**

** Prof. Dr. Tarhan, who stated that the smoking rate in Turkey has exceeded 30%, said, “In Turkey, this rate is 39% for men and 12% for women. It is very high. We are among the countries with the highest smoking rates in the world. There was political determination, NGOs and the media supported it, and it decreased a bit. But we have returned to the old state.” Tarhan draws attention to the brain's brake mechanism and the critical 14 days. **

Üsküdar University Founding Rector, Psychiatrist Prof. Dr. Nevzat Tarhan, evaluated the issue of tobacco addiction. 

## **“They activate the behavioral reward system in the brain...” **

Prof. Dr. Nevzat Tarhan stated that addiction is related to the disruption of the brain's reward system, even calling addiction 'Reward Deficiency Syndrome,' and added the following:

“The person's brain is not satisfied with rewards. Dopamine is the substance that most stimulates the brain's reward center. Dopamine has two important properties; one increases attention and allows focusing on a topic, also called the 'love state.' When the brain focuses only on one subject, it is secreting excessive dopamine. The second property is that it gives a feeling of intoxication. When the brain's reward system works excessively, it evokes a feeling of flying in the person. When this center is highly stimulated, the brain desires more rewards. In fact, it desires unexpected rewards, not just addictive ones. Surprises trigger the normal, accustomed reward system in the brain more, inducing pleasure in the brain.   

We live in an age where pleasure is glorified. Therefore, a life philosophy has emerged in the world that prioritizes pleasure and comfort as life's purpose. It has become a philosophy brought about by modernism. We have become overly worldly, pleasure-seeking, hedonistic, and conformist. For this reason, this also increases dopamine in the brain. It's such that there are substances that trigger dopamine, and behavioral addictions. From social media to gambling, all activate the behavioral reward system in the brain.” 

## **“There is genetic information related to addiction susceptibility” **

Prof. Dr. Tarhan stated that there is genetic information related to addiction susceptibility, “There are two types of genetic mechanisms. One is what we call a causative gene; for example, if a person is genetically predisposed to lung cancer, the DNA in that region starts to degrade when they reach that age. It immediately starts to trigger cancer. The same applies to Alzheimer's. If there is a susceptibility gene, for example, the Alzheimer's gene ApoE4. It is very rarely found. If this gene is present, the person is more likely to develop the disease. It's the same with addiction; if there is a predisposition, when the conditions are right, that gene immediately starts in the person. The state of being activated emerges.”

## **“When nicotism is suddenly stopped, severe headaches, restlessness, and irritability occur”**

Prof. Dr. Tarhan noted that nicotine addiction is defined as nicotism, stating, “Nicotism is related to acetylcholine in the brain. Acetylcholine is a substance related to memory and attention. Smokers smoke when they want to be attentive and focus on something. Concentration and attention in the brain increase. The person focuses more on that topic. At the same time, nicotine has a secondary pleasure-inducing effect. Nicotine also has a stimulating and energizing effect. The effect of nicotine is also present in dopamine. Due to that effect, the person enjoys taking nicotine. E-cigarettes contain only nicotine, not other substances, but they still cause nicotism. Because it causes nicotism, they gradually increase the dose. Long-term use impairs the brain's learning memory. Nicotism is not harmless. When you suddenly quit nicotism, you experience severe headaches, restlessness, and irritability. They become dazed and suffer from inattention.”

## **“Rituals like smoking with tea or coffee trigger smoking”**

Prof. Dr. Tarhan stated that nicotine creates addiction faster than other substances and that there is physiological and psychological addiction in smoking.

Prof. Dr. Tarhan explained that rituals such as taking out and lighting a cigarette, ostentation, and smoking with tea or coffee after a meal create a psychological unity and trigger and remind one to smoke, saying, “These mostly constitute the behavioral addiction dimension. Both combine with physiological addiction.”

## **“The biggest symptom of addiction is irritability”**

Prof. Dr. Tarhan noted that an addicted person being irritable when there are no cigarettes is the biggest symptom, stating, “A person having the urge to light a cigarette as soon as they wake up in the morning is dangerous and harmful. They are coughing, some symptoms of vascular diseases have started, but they still don’t quit smoking. They cough, go to the doctor, the doctor tells them to quit smoking, but they still don’t. In this situation, even though they know it’s dangerous and harmful, they find a way to convince themselves. They say, ‘Nothing will happen to me, I’m not addicted, I can quit whenever I want, nothing will come of this.’ Humans are masters at deceiving themselves.  They don’t take warnings seriously.  They don’t buy a pack to avoid smoking, but continue to smoke by asking others for cigarettes.” 

## **“If one truly wants to quit, it's easy; if not, it's very difficult”**

“The person must first want to quit smoking and make a decision,” said Prof. Dr. Tarhan, explaining that quitting smoking is easy if the person genuinely wants to, but very difficult if they don't.

Prof. Dr. Tarhan stated that there is a 'Behavior Change Model' for quitting smoking, saying, “We look at whether people who come to quit smoking really want to, or if they are doing it for the sake of friends or family. If they say something like, 'This has harmed me, but I can't quit. It's like there's a second person inside me making me smoke,' then the solution is easy here. There are medications that reduce the urge to smoke. These work, and we give them. It's actually an antidepressant, but because that drug reduces cigarette cravings in the brain, it reduces the desire to smoke by about 40-50%. That medication can be given to individuals who are willing.”

## **Critical 14 days to engage the brain's brake mechanism! **

Prof. Dr. Tarhan also noted that the cigarette card method is being used for some people, stating, “We tell them to list why they want to quit smoking. For example, they say, 'I don't want my mouth to smell, I have suspicious findings in my lungs, my blood pressure is rising, I'm coughing.' When the person feels like smoking, they will take out that card and read it, saying, 'I won't smoke for these reasons.' This will engage the brake mechanism in the brain. If they do this for 14 days, this critical period passes.”

Prof. Dr. Tarhan stated that those who succeed in quitting smoking for 7-14 days experience the pleasure of it, and that during this process, individuals can use nicotine gum or patches that gradually reduce the sensitivity of nicotine receptors, adding that these also work.

## **“Smoking rate in Turkey exceeds 30%”**

Prof. Dr. Tarhan stated that the US has been successful in the fight against smoking globally, with an average smoking rate of around 15%, adding, “The smoking rate in Turkey currently exceeds 30%. In Turkey, this rate is 39% for men and 12% for women. It is very high. We are among the countries with the highest smoking rates in the world. There was political determination, NGOs and the media supported it, and it decreased a bit. But we have returned to the old state. Most cigarettes are currently imported. This also results in a tremendous loss of foreign currency.”

## **“Selling cigarettes to under-18s is prohibited, but the starting age is before 18…”**

Prof. Dr. Tarhan noted that smokers have a very high chance of leaving their children orphaned and do not live very long, stating, “In Turkey, selling cigarettes to those under 18 is prohibited, but the starting age for smoking is before 18. This means that among family, environment, and friends, they are encouraged with phrases like 'You've grown up, you're a young man now, light a cigarette.' This is a cultural problem.” 

## **“Adults need to set an example to prevent young people from getting accustomed”**

Prof. Dr. Tarhan also pointed out what needs to be done to protect children and young people from addiction, stating that one should not smoke at home, that one should smoke like a culprit even on the balcony, and that smoking indoors at home is not something that can be solved by laws.

## **“Not using it, quitting, is much easier now…”**

Prof. Dr. Tarhan, pointing out that doctors also need to set a good example, said, “Adults need to set an example to prevent young people from getting accustomed. But even if there is a good example, can it still be smoked? Yes, it is. Peer groups influence it. Advice and conferences are not effective for young people. It's actually much easier not to smoke, to quit now.”

## **“If stress decreases, the need also decreases…” **

Prof. Dr. Tarhan noted that cigarettes are used as an anti-stress measure, stating, “If societal stress decreases, the need for cigarettes also decreases. It is necessary to reduce tension. We are in a geography where we have to live under stress, we have to manage stress. We cannot escape stress, but we can manage it. Problem-solving approaches are needed. If a person wants to quit, the solution is not cigarettes. Addiction therapists are successful in helping people quit smoking. If the brain wants to, it will experience difficulty in the first 7-14 days, and then it will quit.”
