## Özet

Experts state that primary teeth may not fall out on time in everyone, and this should not be considered a simple situation.

## İçerik

Pediatric Dentist Asst. Prof. Şebnem N. Koçan stated that primary teeth remaining in the mouth for a long time can affect the eruption of permanent teeth, tooth alignment, and occlusion. She said, “However, not every primary tooth remaining in the mouth necessarily needs to be extracted. If the tooth is healthy and does not negatively affect the eruption of the permanent tooth or the position of adjacent teeth, in some cases, it can be kept in the mouth for a long time and monitored.” Asst. Prof. Koçan pointed out that early evaluation, especially for primary teeth that remain in the mouth significantly longer than expected for their age, can ensure more controlled management of potential problems.

Pediatric Dentist Asst. Prof. Şebnem N. Koçan from Üsküdar University Dental Hospital made statements regarding the reasons why primary teeth do not fall out on time, the possible effects of their prolonged presence in the mouth, and in which situations they should be retained or extracted.

**A primary tooth remaining in adults is not just ‘a tooth that hasn't fallen out’!**

Asst. Prof. Şebnem N. Koçan stated that it is not correct to say that a primary tooth remaining in the mouth directly impairs jaw development. She added, “However, a primary tooth remaining in the mouth for a long time can affect tooth alignment, the position of adjacent teeth, the eruption of permanent teeth, and the occlusal relationship between teeth.”

Asst. Prof. Koçan pointed out that in cases where a primary tooth prevents the eruption of the underlying permanent tooth or where the primary tooth remains lower than adjacent teeth over time, the normal development and positioning of teeth can be disrupted. She stated, “Adjacent teeth may tilt towards or around the space where the primary tooth is located, the alignment of teeth may change, and occlusal irregularities may arise. The eruption path of the underlying permanent tooth can also be affected; in some cases, it can cause the permanent tooth to be delayed or remain impacted. Therefore, the presence of a primary tooth in an adult should not be considered merely ‘a tooth that hasn’t fallen out.’ The tooth’s position, its relationship with adjacent teeth, its impact on occlusion, and the status of the underlying permanent tooth should be evaluated together. Regular clinical and, if necessary, radiographic follow-up is crucial, especially for primary teeth that remain in the mouth for many years.”

**The cause of a primary tooth not falling out must be investigated! **

Asst. Prof. Koçan reminded that in some cases, a primary tooth not falling out can lead to the underlying permanent tooth remaining impacted or never erupting, and she said the following:

“However, the cause-and-effect relationship here should not be viewed as one-sided. Not every primary tooth that doesn't fall out causes the underlying permanent tooth to remain impacted. In fact, in some cases, the real problem is the developmental absence of the permanent tooth or a disruption in its eruption process, leading to the primary tooth remaining in the mouth. Therefore, in such a situation, clinical and radiographic evaluation must be performed.”

**The gap created when a primary tooth is extracted does not always close on its own!**

“Whether the gap created after a primary tooth is extracted will close spontaneously is different for each patient,” said Asst. Prof. Koçan, emphasizing that many factors are considered, such as the tooth's location, the patient's age, the position of adjacent teeth, the existing dental alignment, and the presence or absence of a permanent tooth underneath.

Asst. Prof. Koçan also noted that the spontaneous closure of the gap is not always an ideal situation. She explained, “Adjacent teeth might tilt or shift into the gap. If a permanent tooth is not present and the primary tooth needs to be extracted, how the gap will be managed should be planned separately. Depending on the patient’s condition, options such as orthodontic treatment, prosthetic treatment, or implants in the future may come into consideration. Therefore, the decision to extract a primary tooth and the management of the resulting gap should not be considered separately.”

**Instead of waiting, thinking ‘it will fall out eventually,’ a dentist should be consulted! **

Asst. Prof. Koçan noted that early evaluation provides a significant advantage when it is observed that primary teeth do not fall out on time during childhood. She stated, “In cases where a primary tooth does not fall out at the expected time, a clinical examination and, if deemed necessary, a radiographic examination can evaluate the presence, development, and eruption direction of the underlying permanent tooth.”

Asst. Prof. Koçan also stated that if an eruption problem or another developmental issue is detected early, intervention can be made at the appropriate time. She said, “This way, it may be possible to support the eruption of the permanent tooth or prevent problems that might arise in tooth alignment. Although intervention is not required for every case, the follow-up and treatment planning for problems detected early can be managed much more controllably. Therefore, especially in situations where one of the primary teeth remains in the mouth significantly longer than expected for their age, I recommend consulting a dentist instead of waiting for a long time with the thought, ‘it will fall out eventually’.”

**Not every primary tooth remaining in the mouth needs to be extracted!**
 />

Asst. Prof. Şebnem N. Koçan recommended that primary teeth remaining in the mouth for a long time should be evaluated for cavities and gum diseases, just like other teeth, and regularly checked. She stated, “Especially if there are conditions such as the tooth’s anatomical structure, difficulty in cleaning, or plaque accumulation around it, cavities and gum problems may develop.”

However, Asst. Prof. Koçan stated that not every primary tooth remaining in the mouth is a tooth that must be extracted, and she concluded her words as follows:

“If the tooth is healthy, maintains its function, does not cause problems for surrounding tissues, and does not negatively affect the eruption of the permanent tooth or the position of adjacent teeth, in some cases, it can be kept in the mouth for a long time and monitored.

Especially in some patients where the permanent tooth is completely absent, retaining a healthy primary tooth in the mouth for as long as possible can provide a treatment advantage. Therefore, the decision should not be made solely based on the patient's age; it should be made by evaluating the tooth's clinical condition, radiographic findings, and the patient's overall dental structure.”
