Prof. Dr. Metin also noted that in necessary cases, epilepsy-related movements can be evaluated with full-night video EEG, and pointed out that even waking up unrested can itself be a sign of a sleep disorder. Prof. Dr. Metin also warned that untreated sleep apnea increases the risk of heart attack and stroke. Neurology Specialist Prof. Dr. Barış Metin from Uskudar University NPİSTANBUL Hospital provided information about the tests used in the diagnosis of sleep disorders and the serious health risks that can arise if they are not treated. The basis of diagnosis in sleep disorders is polysomnography (PSG)! Prof. Dr. Barış Metin, stating that detailed questioning of the patient's complaints and the combined evaluation of objective sleep tests are essential in the diagnosis of sleep disorders, said, 'In cases of suspected obstructive sleep apnea, the gold standard method is polysomnography (PSG), which involves the simultaneous monitoring of brain waves, respiratory flow, oxygen saturation, muscle activity, eye movements, and heart rhythm throughout the night.' Prof. Dr. Metin stated that this test allows for the determination of the apnea-hypopnea index (number of breathing stoppages per hour) and the severity classification of the disease, adding, 'For the differential diagnosis of excessive sleepiness conditions like narcolepsy, the Multiple Sleep Latency Test is used on the day following PSG. This test measures the average time to fall asleep and whether the entry into REM sleep is abnormally early through repeated short sleep attempts. In narcolepsy, a shortened sleep onset latency and the detection of sleep periods beginning with REM (SOREM) are critical diagnostic findings. In other words, a narcoleptic individual begins sleep immediately with the REM phase when lying down for a daytime nap. Additionally, a full-night video EEG examination is also a test used in the presence of suspected epilepsy. In the full-night video EEG test, video recordings and multi-channel EEG recordings of the patient are taken until morning, and it is investigated whether the abnormal movements that occur are epilepsy-related.' The equipment used enables comprehensive and accurate diagnosis of sleep disorders! Referring to the methods used for the PSG test in the sleep laboratory, Prof. Dr. Barış Metin said the following: 'EEG shows brain activity. By looking at the EEG, the physician can understand nocturnal awakenings and sleep stages. Furthermore, EEG has diagnostic value in the presence of suspected epilepsy. Muscle activity recording (EMG) allows for the understanding of movements seen during sleep and abnormal muscle contractions. Respiratory sensors record breathing and enable the recording of breathing stoppages during sleep. Chest and abdominal movement sensors help us understand whether respiratory effort is sufficient. The snoring sensor enables the recording of snoring. ECG measures heart rhythm throughout the night, and abnormal conditions are recorded. Oxygen saturation measures whether the blood oxygen level is sufficient. Video recording is taken to evaluate abnormal movements occurring throughout the night. CPAP/BiPAP devices enable the determination of appropriate pressure and device type for patients requiring assisted breathing devices.' **Even not waking up rested indicates a sleep problem on its own!** Prof. Dr. Barış Metin offered recommendations for individuals experiencing sleep disorders to protect their overall health, saying, 'First and foremost, individuals experiencing severe snoring and breathing cessation at night should immediately consult a sleep disorder specialist.' Emphasizing that sleep apnea is an insidious disease that can cause many serious illnesses if left untreated, Prof. Dr. Metin stated, 'It disrupts fundamental life functions such as attention and concentration, and also increases the likelihood of dangerous conditions like heart attacks and strokes. Excessive daytime sleepiness is also a condition that should be taken seriously. It can be due to sleep apnea, and uncontrollable sleep attacks are also a primary symptom of diseases like narcolepsy. Sleep is a vital function for our lives. We must get quality sleep to have a healthy and successful work, family, academic, and social life.
