Experts state that wisdom teeth that have fully erupted and are in a proper position do not need to be extracted.

However, Oral, Dental, and Maxillofacial Surgery Specialist Dr. Lecturer M. Seda Altop pointed out that problems such as gingivitis can occur in semi-impacted wisdom teeth. She stated, “In this situation, issues like facial abscesses, swelling in the face, pain, and restricted mouth opening may arise, necessitating extraction.” Altop emphasized that radiographic evaluation is required to determine if there is a problem, adding that the decision for extraction can be made after evaluating factors such as the tooth's anatomical relationship with nerves, whether a cyst has formed, and potential damage to surrounding tissues and teeth.

Dr. Lecturer M. Seda Altop, an Oral and Maxillofacial Surgery Specialist at Üsküdar Üniversitesi Dental Hospital, evaluated the issue of whether wisdom teeth should be extracted. Altop stated that even if these teeth are asymptomatic, they can form a sac around them, and each case requires an individual assessment.

**Wisdom Teeth Do Not Always Need Extraction**

Reminding that wisdom teeth are located at the very back of our jaw, Oral and Maxillofacial Surgery Specialist Dr. Lecturer M. Seda Altop said, “Since they are the last teeth to erupt, they often do not find enough space in the jaw arch. Diagnosis and, if necessary, intervention should be planned in the 20s after a general radiographic evaluation.”

Altop provided information on situations where wisdom teeth do not need to be extracted: “Fully erupted wisdom teeth in the mouth; if they are in occlusion (meaning there is a tooth opposite them), in a proper position, not damaging surrounding tissues, and healthy, then there is no need for extraction.”

**Bacterial Danger in Semi-Impacted Wisdom Teeth**

Addressing whether impacted or semi-impacted wisdom teeth require extraction, Oral and Maxillofacial Surgery Specialist Dr. Lecturer M. Seda Altop stated, “In semi-impacted wisdom teeth within the jawbone, gingivitis can develop due to bacterial accumulation between the gum tissue and the tooth. This can lead to facial abscesses, swelling in the face, pain, and restricted mouth opening, thus requiring extraction.”

Altop concluded by pointing out that even fully impacted wisdom teeth within the jawbone can cause problems:

“In completely impacted wisdom teeth, even if asymptomatic, the sac around them can turn into cysts. Therefore, radiographic evaluation is necessary. The decision for extraction is made after assessing its anatomical relationship with nerves, whether a cyst has formed, and any damage to surrounding tissues and teeth.”
