Experts state that for many people, smoking is not just a habit, but has become a way to cope with stress and regulate emotions.

Psychiatry Specialist Lecturer Dr. Alptekin Çetin noted that many people perceive smoking as a coping mechanism, stating, “This perception psychologically complicates the decision to quit.” Lecturer Dr. Çetin pointed out that the reward mechanism created by nicotine in the brain codes smoking as a quick and powerful means of relief, but emphasized that the brain can learn new habits when given the right alternatives. Lecturer Dr. Çetin also shared the information that decisions to quit made with internal motivation are much more permanent than those made under external pressure.
Üsküdar Üniversitesi NPİSTANBUL Hastanesi Psychiatry Specialist Lecturer Dr. Alptekin Çetin provided information on the obstacles in the quitting smoking process within the scope of February 9, World Quit Smoking Day.
**Smoking may have become a tool for emotional regulation!**
Explaining the most common motivational barriers in individuals who want to quit smoking, Lecturer Dr. Alptekin Çetin said, “The physical addiction created by nicotine, the linking of stress coping skills to smoking, the loss of self-confidence caused by previous failed attempts to quit, and the thought ‘if I quit, something will be missing from my life’ stand out as obstacles.”
Stating that many people perceive smoking not just as a substance, but as a support mechanism that regulates daily life, Lecturer Dr. Çetin said, “This perception psychologically complicates the decision to quit. In individuals who say ‘I want to, but I can’t quit,’ a strong addiction, high anxiety, and feelings of inadequacy are usually prominent; the person wants to try but fears failure. In those who say ‘I could quit if I wanted to, but I don’t want to,’ defense mechanisms are often at play; the person tries to maintain a sense of control instead of accepting their addiction. In both cases, the common point is that smoking has become the person's emotional regulation tool.”
**Quitting smoking voluntarily is more permanent than quitting due to external pressure!**
Lecturer Dr. Alptekin Çetin noted that nicotine increases dopamine release in the brain, causing smoking to be coded as a quick and powerful reward, adding, “Over time, the brain associates relief, pleasure, and stress reduction with smoking.”
Pointing out that this coding can be broken by realizing the temporary relief provided by smoking, developing alternative reward and relief methods, and managing nicotine withdrawal with appropriate treatment, Lecturer Dr. Çetin stated, “The brain can learn new habits; the important thing is to offer it the right alternatives. Quitting smoking voluntarily is much more permanent than quitting due to external pressure because behavioral change is supported by internal motivation. Individuals who make their own decision attribute responsibility to themselves rather than external factors when faced with difficulties and manage the process in a more sustainable way. The risk of relapse is significantly higher for smoking quit under duress or solely due to peer pressure.”
**Personalized motivation sources can make the quitting process stronger! **
Lecturer Dr. Alptekin Çetin noted that while health motivation is important, it is not sufficient for every individual on its own, stating, “Especially in young people and long-term smokers, health risks can be perceived as abstract and distant. Personalized motivation sources such as aesthetic concerns, financial gain, being a good role model for children, the feeling of freedom, and increased performance can make the quitting process much stronger.”
**New rituals should replace old ones!**
Lecturer Dr. Alptekin Çetin pointed out that smoking rituals, independent of nicotine, lead to strong conditioning and sabotage the motivation to quit, stating, “Smoking behavior associated with coffee, stress, or breaks becomes automatic. Replacing these rituals with short walks, breathing exercises, sugar-free gum, drinking water, or small habits that engage the mind helps the brain break old associations.”
**The strongest step against addiction is not giving up! **
Underlining that the thought ‘just one won’t hurt’ is one of the most common cognitive traps used by addiction, Lecturer Dr. Çetin concluded his words as follows:
“The brain creates the illusion that control has been regained, but a single cigarette reactivates the addiction circuit and often leads to a rapid return to previous levels of use. Although this thought may seem innocent, it is one of the most common causes of relapse.
To those who have tried to quit smoking many times and failed, I would like to say this: This is not a weakness of will, but an addiction that needs to be treated. Every failed attempt is actually a learning process, and success is possible with the right method, the right support, and the right timing. Not giving up is the strongest step that can be taken against addiction.”
