## Özet

Experts state that conditions such as withdrawal, forgetfulness, and behavioral changes can emerge in old age, adding that these changes may not solely be a natural consequence of aging.

## İçerik

Psychiatrist Asst. Prof. Firdevs Seyfe Şen pointed out that some symptoms might be related to depression, cognitive problems, medication side effects, or different health issues, stating, “As one gets older, it is a very natural process to recall some information later or occasionally struggle to remember a name. However, there are situations that require special attention. If forgetfulness begins to affect a person’s daily life, early evaluation is truly important.”

Asst. Prof. Şen stated that criticizing elderly individuals for their forgetfulness could increase their anxiety and recommended an understanding and supportive approach to families.

Üsküdar University NPİSTANBUL Hastanesi Psychiatrist Asst. Prof. Firdevs Seyfe Şen made statements about how to recognize personality and behavioral changes, forgetfulness, and symptoms of depression in the elderly, how relatives should approach these issues, and the importance of psychiatric treatment.

**It is not correct to attribute significant personality changes solely to old age!**

Asst. Prof. Firdevs Seyfe Şen stated that she frequently encounters reactions such as ‘My mother has changed a lot lately’, ‘she used to be very social’, ‘she doesn't want to see anyone’, ‘she has become very sensitive’, ‘she gets angry very quickly’, ‘she has started to suspect everything’, and added, “As one ages, it can be quite normal for some habits to change. One might become calmer. Their social circle might slightly diminish. However, attributing a significant and newly emerging personality or behavioral change solely to old age is not very healthy. Because sometimes these can be the result of severe depression, cognitive problems, side effects of certain medications, or other medical problems.”

Offering advice to families in such situations, Asst. Prof. Şen said, “‘Instead of saying, “You’ve changed a lot. Why have you become like this?”, it is necessary to approach with more understanding, asking questions like, “I see you’ve been a bit more withdrawn lately. Is there anything upsetting or bothering you?” Because sometimes a change in an elderly person’s behavior can be the first sign of a very serious problem they cannot express to us. Let’s not forget, aging does not mean a complete change in personality. If the change is significant and particularly affects their daily life, this is a situation we need to be vigilant about.”

**If forgetfulness has started to affect daily life, it should not be ignored!**

Reminding that not every instance of forgetfulness in the elderly means Alzheimer’s, Asst. Prof. Şen said, “Forgetting a name, forgetting where one put their keys, sometimes forgetting the location of jewelry or money they hid or put away… None of these immediately mean it is Alzheimer’s.”

Noting that it is a very natural process to recall some information later or occasionally struggle to remember a name as one gets older, Asst. Prof. Şen continued as follows:

“However, there are situations that require special attention. Has forgetfulness started to affect the person’s daily life? Do they ask the same question repeatedly at very short intervals? Do they forget recently occurred events very clearly? Do they struggle significantly with managing their medications or money? Do they have difficulty doing tasks they previously knew very well? Or can they get disoriented in a familiar place?

**Do they sometimes struggle to find their way home when returning?**

These are situations that should not be dismissed as normal complaints and early evaluation is truly important. Because not every instance of forgetfulness is Alzheimer’s, but early detection of symptoms related to forgetfulness significantly eases the lives of both the elderly person and their relatives.”

**Criticizing the elderly for their forgetfulness can increase their anxiety! **

Offering advice to relatives of patients experiencing forgetfulness, Asst. Prof. Şen warned, “It should especially be known that phrases like, ‘Oh, did you forget this too? Look, you forgot again. You just can’t remember,’ are situations that can cause intense anxiety for elderly patients, further accelerate forgetfulness, or lead to the concealment of existing symptoms. Therefore, in such cases, it is always healthier to observe the changes in the patient well, understand the patient thoroughly, and speak with a specialist.”

**Psychiatric hospitalization in the elderly is applied to ensure safety and regulate treatment!**

Asst. Prof. Şen noted that both the patient and their relatives can sometimes be severely traumatized when an elderly individual needs to be admitted to a psychiatric ward, stating, “This situation can lead to a perception that it is a very serious and severe illness, that the hospital stay will be very long, or that the patient will never leave the hospital.”

However, Asst. Prof. Şen explained that hospitalization may be necessary not because the illness is very severe, but sometimes because the patient truly needs intensive care, stating, “We may resort to psychiatric hospitalization to ensure the patient’s safety, regulate treatment, and provide close monitoring. We recommend hospitalization especially in cases of severe depression, patients with suicide risk, sometimes in elderly individuals with psychotic symptoms, or those who struggle to meet their basic needs, to halt deterioration and intervene more quickly. It is important to alleviate the patient’s anxieties with a sentence like, ‘We will observe you for a while to monitor you more closely and regulate your treatment,’ instead of saying, ‘We are admitting you to the hospital because you are very ill.’”

**Withdrawal and forgetfulness in the elderly can be harbingers of depression!**

Asst. Prof. Firdevs Seyfe Şen pointed out that an elderly person usually does not come saying ‘I am depressed,’ and added, “They might not even be very aware of this themselves.”

Underlining that depression in the elderly does not always manifest with obvious sadness as it does in younger individuals, Asst. Prof. Şen concluded her words as follows:

“The person might express themselves with sentences like, ‘I don’t want to do anything. I don’t want to see people. No one should come home. I don’t even want the children to come home. It gets crowded. My head can’t take it. I feel disturbed. I get tired quickly. I am old anyway.’ Often, pain symptoms may be prominent. They might say things like, ‘I can’t think like I used to. I’ve already gotten old. I can’t learn like before.’ There might be sleep disturbances. Appetite can change significantly. A decrease in energy particularly emerges.

Physical complaints, somatic symptoms, increased forgetfulness, and especially withdrawal from social life are very common symptoms we see in geriatric depression. Particularly, newly starting withdrawal and the sudden progression of forgetfulness are symptoms that should be seriously considered for geriatric depression. Sometimes, depression can also manifest with a significant slowing of cognitive functions.
Special attention is required in geriatric depression. Because, unfortunately, suicide risk is more prominent in geriatric depression. Geriatric depression, like many diseases in the elderly, may respond somewhat slower to treatment. Therefore, early recognition and early initiation of treatment are extremely important.”
