## Özet

**Emphasizing that anorexia nervosa, an eating disorder particularly prevalent during adolescence, is spreading globally as an epidemic, Psychiatrist Prof. Dr. Nevzat Tarhan stated that media plays a very important role in the emergence of eating disorders. Tarhan also drew attention to the impact of erroneous family attitudes on the development of the disease, stating, "The perception of a perfect body in the media affects children and young people. It is very difficult for a child who excessively reads fashion magazines not to develop anorexia. It doesn't matter if they are a girl or a boy. Media reduces beauty to physical appearance. However, what makes a person beautiful is not their external appearance, but their character and personality. Unfortunately, we live in an era where only visuality and aesthetic perception are sanctified. Parents also buy into this 'disease' of the media."**

Üsküdar University Founding Rector, Psychiatrist Prof. Dr. Nevzat Tarhan, made evaluations about anorexia nervosa, an eating disorder.

Noting that anorexia nervosa, an eating disorder, is spreading seriously like a pandemic in developed countries,** **Prof. Dr. Nevzat Tarhan said, "Anorexia nervosa is a condition widely seen especially in adolescent girls during adolescence. The reasons for its widespread prevalence are being investigated. Anorexia nervosa existed 50 years ago, but it was not this high. The reasons for this are also being investigated." he said.

## **Anorexia Nervosa Has a Biological Basis**

Noting that anorexia nervosa is not a single-factor but a multi-factor disorder, Prof. Dr. Nevzat Tarhan said, "There is a genetic dimension. For example, there are studies conducted on fraternal twins. Or there are anorexia studies conducted within families, in which anorexia is observed 10 times more frequently. The likelihood of a person having anorexia nervosa, or eating disorders, is 10 times higher if a first-degree relative in the family has it. This also indicates the biological basis of the disease." he said.

Stating that anorexia nervosa also has cultural and psychological dimensions, Prof. Dr. Nevzat Tarhan said, "Additionally, it has a neurobiological and neurogenetic basis. When you create the environment, it emerges immediately. These susceptibility genes are as follows: For example, there are two types of genes related to lung cancer in a person: one is a susceptibility gene, and the other is a causative gene. If a person has what we call a 'causative gene,' meaning the gene that causes it, even if they don't smoke and take good care of themselves by the age of 40, lung cancer suddenly begins. But if there is no causative gene, and only a susceptibility gene, if the person lives healthily, pays attention to what they eat and drink, and doesn't smoke, lung cancer does not start." he said.

## **Incorrect Protein Disrupts Eating Perception**

Noting that there is also a susceptibility gene in eating disorders, Prof. Dr. Nevzat Tarhan said, "However, in some cases, when these environmental, cultural, and psychological conditions are ready, that gene starts, performs excessive gene expression, and produces incorrect protein. What happens with this incorrect protein? The person's eating perception is disturbed. The feelings of hunger and fullness related to eating are disrupted. For example, the person has a fear of gaining weight. There is an extreme desire to lose weight, almost to the point of obsession. The person fears gaining excessive weight. I know of a case where someone, despite weighing 29 kilograms, would stand in front of a mirror and say, 'I am 150 kilograms.' This person is not lying; they perceive it that way. Their brain tells them, 'Yes, you are 150 kilograms.' Yet the person weighs 29 kilograms, their body mass index is clear. In such a situation, when confronted with reality, they do not accept it, they deny it. They disregard it." he said.

Noting that in such cases, the chemistry of the area responsible for body image perception in the person's brain is disturbed, Prof. Dr. Nevzat Tarhan said, "The brain produces faulty thoughts and faulty perceptions. As a result, the person perceives themselves as ugly. Yet, they are much, much more beautiful than that. You look at them, and they say, 'I am very ugly,' and they are afraid to look in the mirror. Even though they have an above-average face and body, they don't like themselves." he said.

## **Excess of Love Sometimes Causes Harm**

Stating that for this condition to arise, a person must have a certain environment and conditions, Prof. Dr. Nevzat Tarhan said, "Just as there is a susceptibility gene for lung cancer, you trigger it when you smoke. Here too, psychologically, sociologically, culturally, and within the family, there are erroneous attitudes of parents that trigger this. There are societal attitudes and errors. Lack of love is an important factor, but it's not enough. Sometimes an excess of love also causes harm. They love too much, but they control too much. Family-based therapies are conducted. In family-based therapies, there is a lot of love, but the mother and father are rigid and controlling. As if the mother is not a mother but a commander. She does not show her affection, instead developing a 'project child.' They are told, 'You must be like this, you must be like that, you must never gain weight.'" he said.

## **Not Eating is the Child's Area for Self-Validation**

Emphasizing that such mothers constantly follow their young children around with a plate in their hand, Prof. Dr. Nevzat Tarhan said, "For that child, not eating is the most important area for self-validation. The feeling of not eating develops to prove oneself to the mother and father. There is both love and anger towards the mother. The mother acts with love, but she combines love with authoritarianism. When love and pressure are combined, it opens the door to many psychiatric illnesses. Sometimes, love and authoritarianism are also behind suicide cases." he said.

## **The Child Enters a War with Their Parents**

Stating that they apply family personality tests within the family, Prof. Dr. Nevzat Tarhan said that these tests help determine and identify the relationship model between the child and the parent. Prof. Dr. Nevzat Tarhan said the following:

"There are parental relationship scales concerning child and parent relationships. For example, they examine whether there is a democratic attitude within the family, whether the parent is authoritarian or totalitarian, or whether there is democratic functioning in the family. If there is democratic functioning in the family, such issues are resolved more easily. If there is a rigid attitude alongside love in the family, the child cannot oppose their parents directly in such situations because they know they are loved. But what do they do? What is the issue that parents care about most? The issue of not gaining weight and looking beautiful. The child chooses that as an area for reaction. The child enters a 'did you eat, did you not eat' war with the mother. Sometimes they can experience this war with the father as well. There are parent-child wars."

## **It is Important to Give Love at the Right Time, in the Right Way, in the Right Form, and in the Right Dose**

Noting that in such situations, the child incorporates their individuality and independence as a meaning of existence, Prof. Dr. Nevzat Tarhan warned, "This can sometimes be an addiction. That's why love is important, but it is important to give love in the right way, at the right time, in the right form, and in the right dose. Giving it in the right dose is important. Love, when it is too much, is like water. If you give too much, you spoil it. If you raise a child with the love meant for 2-3 people, and you keep discipline loose, the child will do nothing in such situations. They will develop a completely rebellious personality."

## **In Distorted Body Perception, the Person Sees Themselves as Ugly**

Noting that there are two types of anorexia nervosa, Prof. Dr. Nevzat Tarhan said, "In the restrictive type, there is a distorted body perception. They see themselves as thin. They see themselves as ugly, they perceive their body image as ugly, they dislike their bodies, and they fear gaining weight. 50 minutes out of every 60 minutes of their lives are spent thinking about this issue. The body mass index of these individuals is also low. These individuals restrict their food intake. They prefer to eat alone so that no one interferes. They eat their food by breaking it into small pieces. They even break up a small pasta dish. They are very meticulous about calorie counting. They also exercise excessively to burn calories. They take diuretics or use laxatives." he said.

Noting that there is also bulimia nervosa among eating disorders, Prof. Dr. Nevzat Tarhan said, "In this, there is also a binge-eating disorder. They eat excessively and then feel tremendous regret. They go to the toilet and vomit. This is a condition that arises from the brain's insatiability for reward. That is why addiction is called reward deficiency syndrome. Here too, the person has an inability to manage this eating urge." he said.

## **Neuromodulation Treatments Are Applied**

Stating that individuals with distorted reality perception in anorexia nervosa are treated as inpatients, Prof. Dr. Nevzat Tarhan said, "These individuals are hospitalized, and new treatment methods are applied. In neuromodulation therapy, stimulation is given to the depths of the brain, stimulating areas related to hunger and satiety, aiming to correct the disturbed chemistry of the person. In a way, the brain's infrastructure is regulated. Areas of the brain specifically related to aesthetic perception are intervened with. In Turkish, it is called Deep TMU therapy.

## **Three-Pillared Treatment is Applied**

Emphasizing that family bonds and family relationships need to be corrected to prevent eating disorders, Prof. Dr. Nevzat Tarhan said, "In some mothers, the narcissism score is so high that they are mothers with high egos. There is love, but the mother has a high ego, she is narcissistic. She sees her child not as her child, but as an extension of herself. She has enslaved the child. There is a master-slave relationship between the child and the mother. As a result, the child cannot individualize or become independent. The child takes refuge in illness." he said.

Noting that all members of the family are taken under treatment in such situations, Prof. Dr. Nevzat Tarhan said, "We address the entire family. That's why we call it a three-pillared treatment here. There's what the doctors will do, what our patient will do, and what our family will do. We also confront the family with the situation. All these processes require time and effort. Psychologists in this field need to be experienced therapists." he said.

Stating that media also plays a very important role in the emergence of eating disorders, Prof. Dr. Nevzat Tarhan said, "This perception of a perfect body in the media affects children and young people. It is very difficult for a child who excessively follows fashion magazines not to develop anorexia. It doesn't matter if they are a girl or a boy. It says, 'This is the perfect body.' Media reduces beauty to physical appearance. However, what makes a person beautiful is not their external appearance, but their charm, character, and personality. The share of physical appearance in making a person beautiful is 20 percent. 80 percent consists of their behavior, temperament, character, speech, and conversation. In other words, their human behaviors. Unfortunately, we live in an era where only visuality and aesthetic perception are sanctified. Parents also buy into this 'disease' of the media." he said.
