## Özet

**Üsküdar Üniversitesi Founding Rector Psychiatrist Prof. Dr. Nevzat Tarhan participated online in the Family and Youth Symposium in the Fight Against Addiction, organized by Samsun Ondokuz Mayıs Üniversitesi. At the symposium, where the multi-faceted issue of fighting addiction with a focus on family and youth was discussed, Tarhan spoke under the title “The Role of Family and Youth in Fighting Addiction.” Stating that addiction is a global problem but is experienced more stormily in Turkey, Tarhan drew attention to an increase exceeding 100 percent among young people within one year. Emphasizing that the fight against addiction requires focusing on root causes and preparing a scientifically based master plan centered on psychological resilience, Tarhan stated that digitalization and social media trigger addiction.  **

## **“We are experiencing this problem more stormily in Turkey”**

Psychiatrist Prof. Dr. Nevzat Tarhan, emphasizing the importance of finding the root cause in the fight against addiction, said: “Addiction is a global problem, but we are experiencing this problem more stormily in Turkey. Statistics unfortunately clearly show this. While the global increase rate is 45 percent, it has exceeded 100 percent among young people in our country within the last year. This is an extremely serious figure. Therefore, in the fight against addiction, it is necessary to act especially by finding the root cause. The detoxification process in addiction treatment, which is the hospital treatment part, is actually quite easy. We hospitalize the patient for 2–3 weeks, overcome the acute period, and apply detoxification. Compared to previous periods, the withdrawal and craving symptoms, which are the first and most agonizing stage of addiction, are no longer painful with new treatments applied today. Patients can overcome this period much more comfortably and painlessly. Despite this, we observe many relapses, or recurrences, after discharge from the hospital. If an addicted patient adheres to treatment for six months after being hospitalized, a 70 percent improvement is achieved. This is an extremely large and important rate. However, the critical point here is to continue that six-month period without interruption. In addiction treatment, there are prevention efforts at individual and family levels, and at school and community levels. In addition, there are policy and environmental level prevention measures, and early intervention approaches within the health system. All these areas must be carried out simultaneously and collaboratively.”

## **“A psychological resilience master plan needs to be created”**

Tarhan stated that the most beautiful and effective treatment consists of efforts made to prevent people from becoming ill: “There are primary, secondary, and tertiary prevention. Primary prevention refers to what needs to be done for healthy people not to fall ill. Currently, the area of primary prevention is largely neglected. Lifestyle studies are extremely important at this point. Indeed, the Ministry of Health is conducting studies on this issue and preparing a master plan for a healthy lifestyle. Similarly, a healthy lifestyle master plan related to addiction needs to be prepared to prevent young people from becoming addicted. The institution that will prepare this master plan is the Ministry of National Education. The concept of healthy living is a key point here. A psychological resilience master plan, addressing how a healthy individual should live, in other words, needs to be created. Because the most beautiful and effective treatment consists of efforts made to prevent individuals from becoming ill. Therefore, primary prevention is extremely important in the fight against addiction. In this context, we are conducting studies aimed at strengthening psychological resilience. However, these studies must be evidence-based, scientifically founded strategies. It is not possible to achieve this goal with traditional methods, by merely giving conferences on values. Value-based education must be provided systematically. These contents can even be conveyed within subjects like physics, chemistry, and mathematics. A comprehensive master plan must definitely be prepared in our education system for this extremely important issue. This is my top priority. Moreover, sufficient material already exists in this area; there is no need to reinvent the wheel. Some of these practices have already started to be implemented in many parts of the world. Acquiring early life skills is extremely important. Similarly, family-oriented programs are also very important. In this framework, strengthening the family has a critical role in the fight against addiction.”

## **“Digitalization has become an important step on the path to addiction”**

Tarhan, addressing the reasons for the increase in addiction, said: “One of the reasons for the increase in addiction on a global scale is the weakening of family ties, the decrease in social support, and the weakening of social control mechanisms. This situation is an important factor in the rise of addiction. Consistent discipline has weakened, effective communication has declined, and approaches to developing emotion regulation skills have changed. Global change and digitalization seriously affect this process. Today, digitalization has become an important step on the path to addiction. At this point, personal awareness is of great importance. Additionally, many people turn to substance use as a method of stress reduction. However, a person who knows stress management and has the ability to remain calm under stress does not turn to substance use in moments of stress. Similarly, an individual with developed social skills does not choose to solve a social problem by using substances when they experience one. Personal attachment is also extremely important here. Attachment is one of the fundamental human needs. When a child is born, they attach to their mother; as they grow, they attach to their environment and social relationships. The human brain has four fundamental desires. First, the human brain desires eternity. Second, it seeks meaning, wanting meaning and purpose. Third, it desires freedom. Fourth, it desires healthy relationships. When these four basic needs are not met, the brain virtually starts to produce noise.”

## **“Conscious families recognize this process much earlier”**

Tarhan stated that there should be a warm and balanced environment within the family: “There is primary prevention for healthy individuals not to fall ill. In addition, there is secondary prevention. Secondary prevention involves identifying risk groups and carrying out preventive work for these groups. At this stage, early detection of risk and the provision of education and counseling services are of great importance. Of course, determining risk requires different and specialized methods. At this point, a multidisciplinary work approach is essential. At the same time, access to addiction services needs to be increased. When access to addiction services increases, problems can be noticed more easily at an early stage. Conscious families recognize this process much earlier. If there is a warm and balanced environment within the family, and if the mother and father are conscious, the problem can be solved before it escalates if addressed in a timely manner. A conscious family can take a clear stance at this point, saying, ‘Either the substance or me.’ In such a situation, if there is a safe environment within the family, the child chooses their family and gives up the substance. Therefore, it is extremely important for the child to love their home and feel attached to their family.”

## **“Social media has a triggering effect on this system”**

Tarhan, drawing attention to the relationship of social media with addiction, said: “There are not only substance addictions but also behavioral addictions. Today, the most common behavioral addictions are no longer cases seen only in certain segments or limited numbers, as in the past. We now have cases where people are hospitalized due to gambling addiction. Digital game addiction, in particular, stands out in this area. We encounter cases where individuals stab their mothers because their mothers turned off the modem. We encounter cases of domestic violence, violence against fathers, and situations leading to the police being called to the house. This picture shows how serious the issue has become. In this context, artificial intelligence also has an impact, especially on adolescents. Regarding age-based use of social media, limits have been introduced in some countries. For example, in Australia, this age limit became 16. In our country too, this issue should no longer be postponed, and measures need to be taken immediately. Social media has an extremely powerful accelerating effect in this process. Social media has a triggering effect on this system, and this must definitely be known. Especially in risk groups, social media use significantly increases the risk. A 100 percent increase within a year… These are data reflected in the press. The current situation clearly shows us that the policies applied in the fight against addiction need to be changed.”
