## Özet

**Üsküdar University Faculty of Medicine launched its “Masters of Medicine Talks” series with the aim of conveying the universal principles of medical science, bioethical norms, and ancient medical practice to future physicians. The first guest of the program, under the title “Brain, Human, and the Art of Medicine,” was Psychiatrist Prof. Nevzat Tarhan, who has completed half a century in his profession. Tarhan, emphasizing the importance of the master-apprentice relationship, professional experience, and observation skills in medicine, stated that the capitalist system first makes people sick and then tries to treat them. Sharing with prospective physicians that prescribing medication to a patient without need is a sin, and that as physicians, they should not fall for this game, Tarhan reminded them that their primary responsibility is to prevent people from getting sick. **

## **Prof. Haydar Sur: “We live in a world where not everyone deserves the word 'master'”**

Dean of Üsküdar University Faculty of Medicine Prof. Haydar Sur delivered the opening speech of the program. Sur stated; “The science and art of medicine value knowledge acquired from scientific written sources. However, even more important is the transfer of masters' knowledge and experience to younger generations through the master-apprentice relationship. Medicine has evolved over centuries through the continuation of this relay race. When the World Medical Association convened in New York in 1914, the debate 'Is medicine a science or an art?' took place, and the declaration stated, 'Medicine and healing are an art that utilizes science most intensely.' Thus, it was emphasized that mastery, manual dexterity, competence, and communication skills hold an important place alongside scientific knowledge. Therefore, we live in a world where not everyone deserves the word 'master'. Our President, Nevzat Tarhan, who has rightfully earned this title throughout his life, founded Üsküdar University and brought it to its current state, opening doors for us to gather in such beautiful environments, and who is a great treasure trove of knowledge, will be the first guest of our ‘Masters of Medicine Talks’ today.”

## **Prof. Nevzat Tarhan: “Life is not long enough to learn everything through trial and error”**

Addressing artificial intelligence with the metaphor of a mirror, President of Üsküdar University, Psychiatrist Prof. Nevzat Tarhan began his words by saying; “There are three types of learning. One is learning by trial and error. You make mistakes, you learn. The average person learns this way, but life is not long enough to learn everything through trial and error. For this reason, smart people benefit from the experiences of others. This is the ideal second learning method. There is also a learning method where some people repeat the same mistake over and over again, never learning. In Anatolia, such types are called 'hödük' (boorish, bumpkin). Those who follow their nose. This is actually a method of not learning. Therefore, benefiting from the experiences of others is the smart thing to do here. Professionally, especially in medicine and healthcare, you learn things from masters that you cannot learn from artificial intelligence. There is a process of mastering, becoming competent, and accumulating experience. For example, if you ask a watermelon seller, 'Is this watermelon unripe?', they will take a look and predict correctly over ninety percent of the time. If you chose, you would only get it right about fifty percent of the time. If you ask 'How did you know?', they say 'I don't know.' In reality, perhaps three thousand watermelons have passed through their hands. Through trial and error, coordinates have formed in their brain from the sound, type, and color of the watermelon. They have mastered it with those coordinates, unconsciously. This is called rational intuition. Intuitive perception occurs in masters of the craft. They have accumulated years of experience, and an intuitive perception has developed. We are in the age of information, but artificial intelligence cannot teach wisdom. Artificial intelligence teaches information in the information age, but artificial intelligence should also be likened to a mirror. You take a mirror, it is smooth, it takes good things from you and reflects them, but artificial intelligence cannot understand, express, or teach morality, conscience, and compassion. Therefore, wisdom and mastery are very important. This is a situation that arises from professional experiences.”

## **“Artificial intelligence mirrors us”**

Stating that artificial intelligence cannot calculate human emotions, Tarhan said; “Regarding artificial intelligence, Hinton says, ‘If we continue at this pace, in 10 or 20 years, all of humanity will be like a 5-year-old child in the face of artificial intelligence.’ This is a very serious risk. A digital, intelligent entity is emerging. Those who protect their information and are information-centric will be eliminated over time. However, those who pursue wisdom will have to add new meanings to themselves. Individuals who are becoming wise will be able to protect themselves against artificial intelligence. Because artificial intelligence does not have a dimension of meaning, purpose, conscience, or morality like becoming wise. Artificial intelligence mirrors us. It can read our facial expressions and gestures, our micro-expressions, our tone of voice, and analyze them. However, it can never calculate our feelings. It can do what it can calculate, but the same is not true for feeling. For example, there are mirror neurons. When we think strongly, mirror neurons are found in the inferior frontal gyrus, in the lower part of the frontal lobe of the brain. Whatever we intend, a corresponding response occurs in the other person's brain. For example, if we act with good intentions towards the other person and inspire trust, the other person's brain releases oxytocin. Oxytocin is the bonding and happiness hormone, and also a hormone related to secure attachment. If we do not inspire trust in the other person, cortisol and noradrenaline are released in the other person's brain via mirror neurons. These are hormones related to fight-or-flight and stress. If they mathematically model mirror neurons, then it's dangerous. If they do this, they will be able to read our thoughts and intentions. Currently, it does not have such an ability to calculate emotions,” he stated.

## **“Humans can evolve by learning”**

Tarhan, explaining Yalom's four fundamental existential anxieties, said: “Irvin Yalom is one of the important living figures in psychoanalysis today. He is a person who has combined philosophy with psychiatry and addressed the philosophical dimension of psychiatry. He talks about four fundamental anxieties. These exist only in humans. First, existential anxiety and the search for meaning. Second, the individual's need for freedom. Freedom is genetically encoded in living beings. For example, let's consider the lion, one of the strongest animals. If its belly is full and no one enters its safe area, its territory, it won't touch you, but if you enter that area, it can attack you. Humans, however, have an unlimited desire for freedom. Humans want to dominate the entire world. Third, humans have a need for social connection. That is, the need to form social bonds and overcome loneliness. And fourth, the need to explain death. Humans are beings who can think about the past and future, evaluate their past, and plan their future. Humans are also aware of the difference between the past and the future. Our brain's algorithms are different in this regard; other living beings live in the moment. Philosopher Heidegger says in existential philosophy, 'Humans are thrown into this world.' Humans cannot be universally explained solely as a product of this world. From the perspective of current evolutionary theory, although intra-species evolution has been proven, the subject of inter-species evolution is an incomplete theory. For this reason, despite having the same structure as humans a thousand years ago, humans are beings who can develop and evolve by learning. A spider or an ant builds its nest similarly today as it did a thousand years ago. Ants and other living beings are no different in this regard. Humans, however, can evolve by learning,” he stated.

## **“A physician must be a very good observer”**

Tarhan, addressing observation in medicine with the example of Avicenna, said; “A physician must be an observer. You need to be a very good observer. One of the most important things in medicine is being a good observer. For example, Avicenna has an example related to this. When the plague outbreak occurred, everyone fled. Avicenna's assistant said, 'Let's flee too, there's a plague outbreak.' Avicenna, however, said, 'We are chief physicians, we cannot escape the epidemic. We must take precautions and help people.' He took his precautions, examined the patients, and was protected from the epidemic. Similarly, the physician Şerafeddin Sabuncuoğlu, who lived in Amasya in the 1350s and also benefited from Avicenna's teachings, said regarding the plague epidemic, 'Eat well, sleep well, greet from a distance.' With these words, he actually draws attention to contagiousness. Not hugging, not embracing, and maintaining social distance... These were also the most emphasized topics during the pandemic period,” he said.

## **“Correct interpretation is the path to wisdom”**

Stating that the ability to reframe develops people, Tarhan said; “The brain has a meaning network. This is called the ‘Default Mode Network’, or DMN. This network is active when the brain is idling. When you are not thinking about anything, this network in the brain's autopilot mode interprets the incoming information. We can make correct or incorrect interpretations. What is important is to learn to interpret correctly instead of misinterpreting every thought that comes to mind. Correct interpretation is the path to wisdom. When you master a profession, you need to abandon automatic learnings and societal prejudices, such as political prejudices, and reframe them. In political psychology, the concept of ‘Automatic Stereotype’ is used in this regard. We also have societal prejudices. For example, when we see someone from a particular ethnic group, we can immediately approach them in a certain way. This attitude reflects in our body language, and the other person feels excluded. These are erroneous records in our brains. If we reframe them, we can change ourselves. The formula for changing ourselves throughout life is our ability to reframe the information we have learned since childhood. This skill develops people, enables them to think outside the box, fosters mastery, and allows them to act differently,” he stated.

## **“As a physician, let's not fall for this game!”**

Emphasizing the importance of preventive medicine, Tarhan said; “Preventive mental health is an important issue that needs to be addressed in both Turkish and Western medicine. Currently, curative medicine is taught, but health and preventive health are not sufficiently taught. However, it is important to also learn how people can avoid getting sick. For example, primary prevention aims to prevent healthy people from falling ill. Secondary prevention aims to make early diagnoses by identifying risk groups. Tertiary prevention, on the other hand, aims to prevent the recurrence of the disease after it has passed. We, however, have focused only on the treatment aspect. Capitalism makes people sick and then tries to treat them. Unfortunately, it doesn't seem to be diminishing, but at least our moral responsibility is to strive for its reduction. Merely prescribing medication and sending patients away turns into a process where the pharmaceutical industry also gets involved. They invent new diseases and try to sell them. This is an understanding imposed on us by capitalist morality. As a physician, let's not fall for this game. Prescribing medication without the other party's need is a sin. The patient needs to be informed to prevent them from getting sick. We have a set of information, but the patient does not. It is our responsibility to convey this information to the patient. If we do not convey this set of information, we will have done our job incompletely,” he stated.

## **“Flaws are part of perfection”**

Reminding that evils are opportunities for development, Tarhan said; “Making mistakes and flaws are actually part of perfection. Chaos theory in quantum physics explains this best. In chaos theory, there is an apparent disorder. We generally know this as the ‘Butterfly Effect’. A meteorologist named Lorenz makes meteorological predictions on a computer. He calculates the probability of rain tomorrow, a week later, or a month later. At one point, he went to drink coffee without deleting the information on the computer. When he returned, he saw that the results of his program had changed. First, he thought his entire work was corrupted. Then he continued from where he left off and this time obtained better results. Upon this, he called it the ‘Coffee Effect’. Meaning he was able to make better predictions after going for coffee. In fact, what we call chaos is also a theorem. There is order within chaos. What appears as chaos to us gains a different meaning when you look at the universe from a metaphysical perspective. Mistakes and evils are also part of perfection. If there were no evils, humans could not develop,” he said.

## **“There is both the freedom to do good and the freedom to do evil”**

Tarhan, who also answered questions from the participants, said that the balance in the universe and free will can be learned through metaphysics. Tarhan stated; “Looking at the law of entropy, or the second law of thermodynamics, cold is explained as the absence of heat, and darkness as the absence of light. Similarly, evil can be considered the absence of good. Here, there is both the freedom to do good and the freedom to do evil. Life on Earth is a test. Rumi has a saying, 'We did not come into this world to live, we are passing through the world by living.' From this perspective, when you make a metaphysical leap, you see that the balance in the universe and free will exist. We are responsible for the evils we commit. Positive sciences are interested in the question 'How?' How does a cell work, how does DNA work, how does the human brain work, how do space and stars operate? They study these wonderfully and make new discoveries. Philosophy, religion, and other meaning sciences are more interested in the question 'Why?' than 'How?' They ask, 'Why does it exist?' For example, if you ask what this pen is, you say, 'This part is yellow, that part is red, it's a pen of this brand,' but if you ask 'Why does the pen exist?', you answer, 'It exists for writing.' When you pick up a book, you should also ask, 'Why does this book exist?' The point is not whether the book is beautifully or elaborately written; there is a message the author wants to convey here. The real question to ask is what that message is,” he concluded his words.

At the end of the talk, Tarhan signed and gifted his books to the students who asked the three best questions.

The organized talk concluded with a group photo session.
