## Özet

**President of Üsküdar University, Psychiatrist Prof. Nevzat Tarhan, attended the "1st Neuro Cell Molecular Research Congress" organized by Kastamonu University Faculty of Medicine online. At the congress, which brought together scientists and researchers from different countries, Tarhan, who gave a presentation titled "What Does Molecular Neuropsychiatry Promise?", made evaluations regarding personalized medicine, pharmacogenetics, neuroscience, and artificial intelligence. Stating that wrong treatment is the most expensive treatment, Tarhan expressed that pharmacogenetic tests are guiding in personalized treatment. Emphasizing that future drugs will be biotechnological drugs, Tarhan pointed out that psychiatry, neuroscience, and molecular genetics should be considered together. Tarhan also stated that artificial intelligence will not replace a physician, but can even facilitate physicians' work.**

## **“Current developments in biotechnology necessitate a new orientation”**

Tarhan, drawing attention to the role of molecular neuropsychiatry in understanding and treating central nervous system disorders, said: “This study aims to evaluate the promising paradigms offered by molecular neuropsychiatry in understanding and treating complex central nervous system disorders. Traditional psychiatric and neurological approaches largely rely on clinical observation and symptom-based diagnostic frameworks, but current developments in biotechnology necessitate an orientation towards etiology-focused biological interventions.”

## **“Transforms into a personalized, target-oriented, and evidence-based biological medical discipline”**

Tarhan also stated that molecular neuropsychiatry combines different scientific methods, underlining personalized treatment options. Tarhan said: “Molecular neuropsychiatry integrates genomic, pharmacogenetic, neuroinflammatory processes, and synaptic plasticity pathways into clinical practice to elucidate the underlying pathophysiology of neuropsychiatric conditions. This field combines neurotechnological methods such as quantitative EEG and functional brain mapping with biomarker discovery and genetic profiling to offer personalized treatment options. Current clinical data show that biomarker-guided personalized medicine (precision medicine) significantly increases treatment success in resistant psychiatric cases and reduces the risk of side effects thanks to optimized pharmacotherapy. Consequently, molecular neuropsychiatry transforms psychiatric disorders from being merely symptom-focused diagnostic clusters into a personalized, target-oriented, and evidence-based biological medical discipline.”

## **“When we perform incorrect treatment, this is the most expensive treatment”**

Tarhan, stating that correctly defining the concept of expensiveness is very important, said: “Personalized medicine has two pillars. One is personalized medicine itself, covering aspects like nutrition, health, sleep, and sport. The other is Precision Medicine, which is the treatment area. It applies to all healthcare professionals; an alliance is formed if they work together for the patient's well-being. The therapeutic relationship is said to begin from the first encounter between the healthcare professional and the patient. When we perform incorrect treatment, this is the most expensive treatment. The most expensive treatment is actually the most ineffective treatment, because the patient will not get better, it will prolong, progress further, and become chronic. Therefore, this definition of expensiveness needs to be correctly defined.”

## **“Correct drug, correct dose, correct path are important”**

Prof. Nevzat Tarhan, stating that pharmacogenetic tests are guiding in personalized treatment, mentioned that drugs can produce different results in individuals. Tarhan said: “The new era for personalized treatment began in 2003. Correct drug, correct dose, correct path are important. When we do a DNA test, some individuals require a lower dose, while others need double the dose. For some, another drug is needed. DNA tests, pharmacogenetic tests guide us in identifying this. Safety and effectiveness are important; there are four types of human responses here. When you perform a DNA test, you separate the safe and effective group. This, unfortunately, constitutes one-fourth. Safe but ineffective, they are taking medication in vain. Or an individual is unsafe and ineffective; such a group also exists, where it causes side effects and offers no benefit. The other is unsafe but effective. Here, toxicity emerges with its side effects. Pharmacogenetic profiles guide us in distinguishing these, and once we identify an individual's pharmacogenetic profile, we can use that genetic profile for their lifetime. Meaning, whenever they visit a specialist, they can present that file like a pharmacogenetic ID. There are four types of people when it comes to treating diseases. Those who respond normally to a normal dose, those who respond to a low dose, those who respond to a high dose, and those who do not respond and require alternative treatment. This guides us in responding to it.”

## **“The genome revolution happened in 2003”**

Tarhan, stating that Precision Psychiatry is shaped by advancements in neuroscience and genomics, said: “Precision Psychiatry here is the personalized treatment method we use in psychiatry. The dawn of the century we are in opened with the beginning of the neuroscience revolution. The genome revolution happened in 2003. And in 2013, the neuroscience, neurotechnology revolution occurred. Both were announced together with an institution similar to the American TÜBİTAK. In the years around 1997, we first brought the combined use of neuroscience and psychiatry to Turkey, and since then, from the 2000s onwards, the era of treating conditions using brain connectomes began. Here, genomes, similarly, all modern approaches, including genetic imaging and interventional techniques, became part of personalized treatment.”

## **“Our field is determined more by our history than our future”**

Tarhan, emphasizing the need for a competent workforce for the implementation of Precision Psychiatry, stated that the relationship between psychiatry and neuroscience should be strengthened. Tarhan said: “The successful implementation of Precision Psychiatry requires a skilled and ready workforce. Our field is determined more by our history than our future. This is the confession of psychiatrists. If those who approach psychiatry keep their distance from neuroscience and genetics, they want to progress with the history of psychiatry. This is respectable, but in this case, others will do it, and we will imitate. In 5-10 years, we will do what they say.”

## **“Researcher clinicians have moved to multidisciplinary studies”**

Tarhan, pointing out that psychiatry, neuroscience, and molecular genetics should be considered together, emphasized the importance of multidisciplinary work. Tarhan said: “Regarding psychiatry, neuroscience, and molecular genetics, in 2015, Ross and colleagues conducted and published a study titled 'The Critical Role of Education'. Here, the increasing complexity of medicine presents an important landscape. The work of each clinician and researcher in small but increasingly smaller cells has led to specialization. That is, researcher clinicians have moved to multidisciplinary studies. The study states that the number of departments with sufficient expertise in neuroscience is found in very few programs. It states that expertise in neuroscience should be independent of the skills required to teach a clinical audience. Therefore, there is such a recommendation regarding the use of molecular biology and genetics in psychiatric practice, stating that resources should be applicable to cover a wide range of topics and keep pace with new discoveries.”

## **“Artificial intelligence does not replace a physician, it will facilitate our work”**

Tarhan, stating that they aim to utilize artificial intelligence in personalized treatment with their patented "NP Model", said: “We have a patented model called 'NP Model'. In this model, we will digitalize everything from the patient's admission and record brain signals. Based on the signals in the brain, neural networks, genetic algorithms, and the individual's medical records, the physician will input this information into artificial intelligence and check which biotype it falls into. This is personalized treatment. For example, here, medication, magnetic stimulation therapy, neuromodulation therapy, psychotherapy are more important and will facilitate our work together with artificial intelligence. We are always asked, 'How will artificial intelligence affect the medical profession, how will it affect the psychology profession?' It will facilitate our work. It will not replace a physician or a psychologist, but it will greatly contribute to us performing therapies faster and assisting them. In the 1960s, the question was, can machines think? The current question is, can machines feel? The biggest question right now is, can machines feel? But of course, it's still too early to answer that question.”
