## Özet

Experts state that any factor increasing intra-abdominal pressure can trigger the onset of hemorrhoidal disease, and activities such as excessive constipation or diarrhea, prolonged sitting on the toilet, increased intra-abdominal pressure during pregnancy, a sedentary lifestyle, or prolonged sitting can trigger the onset of hemorrhoidal disease.

Prof. Dr. Ahmet Murat Koca, highlighting the importance of timely treatment for hemorrhoids, said, “We should pay attention to the water we drink, eat fibrous foods, and take measures to resolve conditions like constipation or diarrhea. If we are overweight, we should try to reduce it. We may need to adopt an active lifestyle. This way, we can take preventive measures.”

Üsküdar Üniversitesi NPİSTANBUL Hastanesi General Surgery Specialist Assist. Prof. Dr. Ahmet Murat Koca provided information about hemorrhoidal disease, also known as ‘basur’ among the public, and its treatment.

Many causes contribute to the onset of hemorrhoidal disease

Assist. Prof. Dr. Ahmet Murat Koca noted that hemorrhoids are small cushions located in the anal region that help prevent the leakage of gas and fluid from the anus. He added, “However, when these cushions lose their properties, they emerge like a type of varicose veins and can cause hemorrhoidal disease. There are many reasons for the development of hemorrhoidal disease.”

Excessive constipation or diarrhea triggers hemorrhoids

Emphasizing that any factor increasing intra-abdominal pressure can trigger the onset of hemorrhoidal disease, Assist. Prof. Dr. Ahmet Murat Koca said, “These factors include: irregularities in bowel movements such as excessive constipation or diarrhea, prolonged sitting on the toilet, being overweight or obese, increased intra-abdominal pressure during pregnancy, poor dietary habits, a sedentary lifestyle, prolonged sitting, or activities like constant heavy lifting. It should be remembered that these factors can trigger the onset of hemorrhoidal disease. Therefore, adopting a healthy lifestyle and keeping these factors under control is important.”

Sedentary lifestyle must be abandoned

“In the treatment of hemorrhoidal disease, medical treatment or surgical intervention may be applied, but the most important thing is to take certain measures to prevent the disease from developing,” said Assist. Prof. Dr. Ahmet Murat Koca, adding that if the disease does occur, lifestyle issues must be addressed in addition to medication use. He stated, “For example, if there is a sedentary lifestyle, we must be active; if there are problems in our diet, we must correct them; if we are overweight, we must lose weight; if we have a habit of sitting on the toilet for a long time, we must change it. First and foremost, taking these preventive measures is important. We must include fibrous foods in our diet.”

If medication is ineffective, surgical intervention is required

Noting that hemorrhoids can be divided into first, second, third, and fourth stages, Assist. Prof. Dr. Ahmet Murat Koca continued:

“First and second-degree hemorrhoids generally do not protrude much and can be treated with medication. However, if medication is ineffective or the condition progresses, surgical intervention may be necessary. Third and fourth-degree hemorrhoids usually require surgical intervention. Surgical intervention methods can include laser treatment or classical surgical methods. To decide which treatment method is appropriate, the disease should be thoroughly evaluated, and a treatment plan should be formulated accordingly.”

What is the impact of hemorrhoids on quality of life?

Assist. Prof. Dr. Ahmet Murat Koca stated that in terms of the symptoms it presents, if there is an abnormal condition, it can cause pain, saying, “There might be a palpable lump, itching due to fluid leakage, or bleeding. These conditions can make our lives uncomfortable. It is necessary to take measures to eliminate these symptoms. Correcting deficiencies in our lifestyle and timely treatment are important. We must pay attention to the water we drink, eat fibrous foods, and take measures to resolve conditions like constipation or diarrhea. If we are overweight, we should try to reduce it. We may need to adopt an active lifestyle. This way, we can take preventive measures. Afterwards, if medication is required, medication treatment is started, or if surgical treatment is required, intervention is performed.”

Treatment with local anesthesia is possible during pregnancy

Explaining that hemorrhoids are also considered a type of varicose vein, similar to how varicose veins increase in the legs or appear in many parts of the body due to pregnancy, Assist. Prof. Dr. Ahmet Murat Koca said, “This condition can cause hemorrhoids to appear due to the pressure exerted by the baby inside the abdomen in that region, and its effect on circulation. These conditions, as long as pregnancy-related hemorrhoids are not major complicated situations, can usually be resolved with local medications and medical treatments. In the post-pregnancy period, the pregnancy process should be monitored, and if it is in an advanced stage, an appropriate treatment plan should be formulated. However, sometimes, thrombi, i.e., thrombosed hemorrhoids, which are a complication of hemorrhoids, can appear during pregnancy. In this case, it can be treated by removing the clot from the hemorrhoidal packet with local anesthesia. This procedure can be easily performed with local anesthesia.”

More common after age 50

Stating that hemorrhoidal disease can be seen at any age, Assist. Prof. Dr. Ahmet Murat Koca concluded, “However, it is less common before the age of 20. It is generally seen more frequently after the age of 50. However, the age range can vary depending on lifestyle and life factors.”
