## Özet

**As rescue efforts continue after the severe earthquakes in Kahramanmaraş, Psychiatrist Prof. Dr. Nevzat Tarhan pointed out the importance of psychological first aid. Tarhan noted that individuals experiencing earthquake shock primarily want their feelings to be understood, emphasizing the importance of feeling safe during this period. Reminding that the most common mistake is to try to make sense of the situation, and that now is not the time for such interpretation, Tarhan stated that periods of protest and passivity emerge after the initial shock. Tarhan added that this situation can last between 6-8 weeks, drawing attention to the importance of the individual experiencing their pain and successfully overcoming this emotion.**

## **Earthquake Victims Need Their Feelings Understood and Security**

Üsküdar Üniversitesi Founding Rector, Psychiatrist Prof. Dr. Nevzat Tarhan, made evaluations regarding post-earthquake traumas.

Prof. Dr. Nevzat Tarhan noted that natural disasters like earthquakes always have a widespread impact, affecting not just specific individuals but broad societies, and stated that over 18,000 people lost their lives in the two severe earthquakes that occurred consecutively in Kahramanmaraş and caused extensive damage in 10 provinces. Prof. Dr. Nevzat Tarhan also noted that a nationwide aid mobilization was launched for the region following the earthquake, which was also monitored by the global public, and drew attention to the importance of psychological first aid for those who experienced the earthquake.

## **Earthquake Victims Want to Feel Understood**

Prof. Dr. Nevzat Tarhan noted that it is important for earthquake victims to feel understood first and foremost, stating, “In an earthquake situation like this, earthquake victims need to feel that they are understood. As a society, we need to make them feel that we deeply understand the pain and hardship of the people in that region. We already feel this as a society; we are a highly empathetic society. We are a society with empathy so strong that we could almost sit and cry with someone who is crying. This is actually a beauty stemming from our culture.”

## **The Mistake of Interpretation Should Be Avoided!**

Prof. Dr. Nevzat Tarhan stated that the earthquake has a traumatic effect, saying, “Just as we have not forgotten the August 17 Marmara earthquake, this earthquake will not be forgotten either. The most common mistake made here is the error of interpretation. Now is not the time to interpret this earthquake. This situation is similar to this: when a fire breaks out, you don't ask why the fire started, why it happened that way, why you did this, or why you didn't take precautions. First, the fire is extinguished, cooled down, and then the causes are discussed. In an earthquake, just like in other crises, interpretation should not be made; instead, focus should be on the immediate actions that need to be taken. As a society, everyone should focus on what they need to do at that moment.”

## **There is a Need to Feel Safe**

Prof. Dr. Nevzat Tarhan stated that the greatest need in such situations is security, saying, “The most primary need for people in the region is the need for security. They need to feel that they are not alone. They need to feel safe. This need alleviates their sense of panic; secondly, they have a need for information. Uncertainty and silence further increase anxiety and fear.” he warned.

## **Shock is Experienced First in a Crisis**

Prof. Dr. Nevzat Tarhan pointed out that individuals go through stages during a crisis, stating, “In the first stage, there is a shock period. During that shock period, we sometimes see people wandering around staring blankly. Individuals whose relatives are trapped under the rubble may be seen walking around indifferently. This is actually due to the intensity of the shock. The brain blocks itself and desensitizes the person. If the person didn't do this, they would lose their mind. The brain secretes excessive cortisol. With that cortisol, the pathways in the brain become blocked. This is similar to: when a person breaks their arm, the pain is so severe that you cannot move your arm. In fact, it should not be moved because it is dangerous, and the body reflexively stops its movement. Similarly, the brain blocks itself. Some individuals experience this shock period.” he said.

## **Cognitive Stagnation and Blockage are Experienced**

Prof. Dr. Nevzat Tarhan stated that people might exhibit unusual behaviors during the shock period, saying, “Those going to the earthquake region for help should keep this in mind. This is a rule of psychological first aid; the first thing someone going to intervene in a crisis moment like a natural disaster should know in psychological first aid is the cognitive sluggishness and state of blockage in a person during that shock period. People need to accept panicked behaviors. They will not correct a person's mistaken action with advice. In such situations, they will hold the person's hand, say, ‘We are with you, we want to help,’ and perhaps even quietly standing by them and waiting might be effective.” he said.

## **Can Last 6-8 Weeks**

Prof. Dr. Nevzat Tarhan stated that after the shock period, some individuals might experience a protest period, saying, “During this period, individuals may react with statements like ‘Why did this happen, why did this happen to me, I don't deserve this.’ This period is followed by a period of passivity. In some individuals, passivity may be seen as entering a depressive mood. The person feels as if they have regressed to childhood. Afterwards, a recovery period emerges. All of these generally occur within a period of 6-8 weeks.” he said. 

## **Inability to Sleep is Observed in Post-Traumatic Stress Disorder**

Prof. Dr. Nevzat Tarhan noted that if all these processes continue for longer than 6-8 weeks, one can then speak of post-traumatic stress disorder, stating, “Individuals with post-traumatic stress disorder cannot sleep at night because they are constantly thinking about it or seeing the event in their dreams. There is what we call re-experiencing, or flashbacks. The person feels as if the moment of earthquake shock, the moment of shaking, is happening again and again.” he said.

## **The Person Needs to Succeed in Overcoming This Pain**

Prof. Dr. Nevzat Tarhan stated that the person is constantly vigilant and may experience frequent startles, saying, “There are startles at sudden sounds, and they constantly talk about this issue. They talk about this for 50 out of 60 minutes. If this situation continues for longer than 6-8 weeks, help is needed. It is natural for the situation to continue for 6-8 weeks; medication should not be given during this period. Medication should not be given unless the situation is extremely severe. This is pain, and the person needs to experience and succeed in overcoming this pain.” he said. 

## **Feelings of Uncertainty Must Be Eliminated**

Also noting that meeting people's basic needs and eliminating feelings of uncertainty are very important in disaster and crisis situations like earthquakes, he said, “Uncertainty here creates excessive anxiety and fear about the future. The reactions given here can vary from person to person. Some withdraw, some talk constantly, trying to explain the same thing over and over. Some people start to think that living is empty and meaningless.”

## **Careful Approach is Needed for Children, the Elderly, and People with Disabilities**

Prof. Dr. Nevzat Tarhan also noted that approaching disadvantaged individuals is important, saying, “Children's reactions are different, the elderly's are different, and the reactions of people with disabilities are different. In such situations, the disadvantaged individuals here are children, the elderly, the sick, people with disabilities, and those in hospitals. We must not overwhelm the other party with our gazes and behaviors; constantly hovering, trembling over them, giving them such special treatment creates a kind of feeling of gratitude and embarrassment. The other party may feel bad. A balanced attitude should be adopted here. Care should be taken not to hurt the person being helped and not to make them feel bad.” he warned.

Prof. Dr. Nevzat Tarhan also noted that the topic of the earthquake should not be brought up with earthquake victims unless they themselves wish to discuss it.
