## Özet

**Eating disorders naturally begin in adolescence when physical appearance comes to the forefront. As children socialize, they attach more importance to their physical appearance due to environmental and peer factors. Prof. Dr. Nevzat Tarhan points out that parental influence is much more prominent in these types of disorders, attributing Anorexia Nervosa, Bulimia Nervosa, and other restrictive eating disorders to unfulfilled emotional needs during childhood. Tarhan states that behind eating disorders lies body image disorder, meaning children who dislike their own bodies. He added that because these individuals have distorted perceptions of weight gain and body image, they perceive themselves as overweight even when they are thin, to the extent that they can become so obsessed they measure pasta by the centimeter before eating it. **

Just as diseases occur when protein, carbohydrates, or vitamins are not consumed, people also become ill when emotional needs are not met. For instance, in Anorexia Nervosa, a type of eating disorder, individuals perceive themselves as beautiful despite weighing 29-30 kilograms and experience a fear of gaining weight. Üsküdar University Founding Rector, Psychiatrist Prof. Dr. Nevzat Tarhan provided important information on various eating disorders and their causes.  

## **They Are So Obsessed They Measure Pasta by the Centimeter Before Eating It**

Tarhan stated that Anorexia Nervosa patients constantly restrict their food intake due to anxiety about weight gain, spending 50 out of 60 minutes of their time focusing on physical appearance. "They can even subject themselves to a destructively severe hunger that could lead to death. Behind eating disorders are body image disorders, meaning children who dislike their own bodies. Because their perception of weight gain and body image is distorted, they perceive themselves as overweight even when they are thin. They constantly look in mirrors, prefer to eat alone, hide their food, experience frequent mood swings, avoid socializing, and particularly dislike entering eating environments. They are overly focused on the nutritional content of food. They are so obsessive that they almost measure pasta by the centimeter before eating it. In the bulimic type of Anorexia Nervosa, there are compensatory behaviors such as purging after eating. They engage in excessive exercise. Today, it has become very widespread, especially in areas influenced by Western modernity. Although less common in Eastern societies, its prevalence has been globally affected by the influence of communication and media," he said, adding that 15% of these cases result in death.

## **Bigorexia in Men**

“Bigorexia is also known as the male counterpart of Anorexia in boys. We can define it as the feeling of 'I must have a perfect muscular physique, I must stay thin, and I must be lean.' This is more of a male-type eating disorder. These individuals spend most of their time in the gym, exercising to death and taking supplements.” 

## **Eating Disorders Appear in Children Whose Emotional Needs Are Not Met**

Tarhan emphasized that the mother-child relationship is particularly prominent in these disorders, stating, “Because food is the most glorified topic at home, with the 'did you eat, did you not eat' pushing, the child gravitates towards the exact opposite. When we look at the families of individuals who experience emotional neglect during childhood, we see them chasing after the child with a plate, feeding and giving them drinks, but no emotional transfer occurs in the child. In a child whose emotional needs are not met, both love and anger develop towards the mother, creating an anxious rather than secure attachment. In such situations, to manage their anger, they internalize their anger towards the mother in their body and do not show it explicitly. They punish themselves, essentially taking revenge on their mother. The more the mother says 'eat,' the child doesn't eat. They make their mother angry, and they enjoy this situation. They express the anger they cannot show directly in a passive-aggressive way by making their mother angry. This child, who wants to play and talk with their family, fills the feeling of emptiness within them with food, and sometimes this attitude even leads to obesity. Some even begin to perpetrate violence against their parents after adolescence. Even if the child has no criminal tendencies, they react like a cornered animal.”

## **If You Look Good, You Are Valuable; If You Don't, You Are Worthless**

Prof. Dr. Nevzat Tarhan stated that physical appearance is especially considered a measure of worth in girls; “If they are overweight, shame is instilled in the child, and a feeling of worthlessness is taught by saying 'you look ugly, you don't take care of yourself.' In Japanese societies, success is the measure of value. Because it is taught that 'if you are unsuccessful, you are worthless,' those who fail may even commit suicide. In Western societies, however, it is taught that 'if you are not beautiful, you are worthless.' Social media also greatly influences the perception of physical appearance. Especially the expectation of receiving likes becomes directly proportional to a person's self-worth. Such as 'If I get many likes, I am valuable; if I get few, I am worthless...' Eating habits are closely related to looking good on social media. It's a kind of 'fetishism' that sanctifies looking good. If you look good, you are valuable; if you don't look good, you are worthless...”  

## **Brain Chemistry Is Corrected First **

Tarhan emphasized the necessity of a three-pronged treatment approach for Anorexia Nervosa;  “There are things the doctor, the patient, and the family need to do for treatment. First, we look at the individuals' brain functions. Because the brain's body perception is disturbed, the feeling of hunger in the brain also disappears, and chemicals like serotonin and dopamine decrease. Hormones and the immune system become dysfunctional, weaken, and they frequently get sick. They do not feel hunger, fullness, or the need to eat. Magnetic therapies are applied depending on the severity of the illness. After the brain chemistry is corrected and reaches a certain point, family therapies then begin,” he said, providing information about the treatment approach.  

## **Anorexia Nervosa Is a Family Disease**

Tarhan explained the importance of the family in treatment as follows: “When we look at the family side, they make mistakes with good intentions. We also work on this situation. Anorexia Nervosa is a family disease. If we don't address the family and only focus on the individual, the disease recurs when the person enters the same environment. If the family is open to change, we achieve very easy, very good, and rapid results. I have seen even the most severe cases of Anorexia recover with a 6-month treatment. Some patients even start saying 'Why did I think such absurd things?' after some time into treatment.” 

## **One Should Not Fall into the Belief That Anorexia Nervosa Is a Fatal Disease**

Concluding his remarks by stating that Anorexia Nervosa is a chronic disease that does not resolve on its own and requires a patient treatment process, Üsküdar University Founding Rector, Psychiatrist Prof. Dr. Nevzat Tarhan said, “When patients are left untreated, they turn into individuals living lost lives. If the person does not receive treatment, there is a possibility of recurrence. We follow up for 2-5 years after treatment. If the treatment continues its normal course, the brain automates normal living, thus eliminating the possibility of recurrence. The probability for the patient becomes the same as it is for other people. Therefore, one should not fall into the belief that 'Anorexia Nervosa is a fatal disease.' If treatment is carried out exactly as prescribed, the person does not develop incorrect coping strategies to overcome negative emotions related to the illness. They learn to manage self-harming emotions such as anger and hostility. If they experience feelings of distress, fear, loneliness, or shame, we apply a therapy technique called 'Cognitive Emotion Regulation' method. With this technique, the person learns to manage their negative emotions correctly, and the disease does not recur.”
