## Özet

Experts state that tooth sensitivity most commonly arises from weakening of the enamel layer and gum recession, and that bruxism is among the significant causes accelerating this process.

Asst. Prof. Kübra Güler, a Periodontology Specialist, noted that incorrect brushing habits and neglect of oral hygiene can also increase sensitivity, and said, “Just as there can be periods of increased sensitivity due to hormonal changes, a similar increase can be observed when oral hygiene is neglected.” Asst. Prof. Güler, who shared recommendations that can help reduce tooth sensitivity, emphasized that professional support must be sought if the problem persists.

Üsküdar University Dental Hospital Periodontology Specialist Asst. Prof. Kübra Güler provided information about the causes, mechanism of formation, triggering factors, and measures to reduce tooth sensitivity.

**Tooth sensitivity arises from the weakening of the enamel layer! **

Stating that tooth sensitivity is defined as a sudden and short-term pain or discomfort in the teeth in response to external stimuli, Asst. Prof. Kübra Güler said, “To better understand this condition, an explanation can be given using the example of skin. When a wound occurs on the skin, sensitivity develops in that area, and the person avoids touching that area. Similarly, when any damage or opening occurs in the enamel layer, which is a protective structure in the teeth, sensitivity arises.”

Reminding that the enamel layer, which forms the outermost layer of the tooth, is a hard structure that protects the tooth against external factors, Asst. Prof. Güler said, “However, as a result of conditions such as weakening or erosion of this layer or gum recession, the unprotected areas of the tooth become sensitive to external stimuli. In this case, especially when cold, hot, sweet, or spicy foods and drinks are consumed, a sudden tingling or discomfort is felt in the teeth. In short, tooth sensitivity is pain sensitivity that develops against external factors, resulting from the weakening or loss of the tooth's protective enamel layer.”

**Bruxism makes teeth sensitive to external factors!**

Noting that teeth grinding and clenching hold a significant place among the most common causes of tooth sensitivity, Asst. Prof. Kübra Güler said, “Bruxism occurs especially during sleep or in moments of stress, when teeth involuntarily make excessive contact with each other. This constant and strong contact can lead to micro-cracks and abrasion on the enamel, which is the protective outer layer of the teeth, over time.”

Pointing out that this damage to the enamel layer causes the tooth to be unprotected against external factors, Asst. Prof. Güler continued as follows:

“Furthermore, gum recession can also develop due to bruxism. With gum recession, the tooth root surface becomes exposed, and these areas become sensitive because they are not protected by the enamel layer. Consequently, both abrasions in the enamel layer and gum recession expose the normally protected surfaces of the tooth to external stimuli. This situation, in turn, causes sensitivity and pain in the teeth, especially when they come into contact with cold, hot, sweet, or spicy foods.”

**If sensitivity affects daily life, professional support should be sought!**

Stating that tooth sensitivity is a condition that can increase periodically, Asst. Prof. Kübra Güler said, “Just as there can be periods of increased sensitivity due to hormonal changes, a similar increase can be observed when oral hygiene is neglected. Additionally, in patients with teeth clenching problems, tooth sensitivity can significantly increase, especially during periods when clenching intensifies.”

Asst. Prof. Güler stated that professional support is generally not needed in cases where tooth sensitivity is noticed periodically and decreases over time, but added, “However, if the sensitivity is progressively increasing, if it has started to be intensely affected by external factors such as cold and hot, and if it has reached a level that negatively affects the person's eating and drinking habits, then professional support must be sought, and the necessary sensitivity treatment should be applied.”

**If oral hygiene is not performed regularly, gum infection can develop!**

Emphasizing that the daily oral care routine is an important factor directly affecting tooth sensitivity, Asst. Prof. Kübra Güler said, “If oral hygiene is not performed regularly, infection can develop in the gums. Inflammation and infection of the gums, in turn, make the teeth more sensitive. Therefore, daily oral care must be maintained without interruption.”

However, Asst. Prof. Güler also stated that incorrect oral hygiene practices can increase tooth sensitivity, and said, “Especially brushing teeth with too much pressure or using hard toothbrushes can lead to gum recession and micro-fractures in the enamel layer. This situation also causes an increase in tooth sensitivity. Therefore, it is important to brush teeth with the appropriate technique and to regularly continue the daily oral care routine.”

**To reduce tooth sensitivity, the underlying cause must first be addressed!**

Asst. Prof. Kübra Güler, who made suggestions that can help reduce tooth sensitivity, concluded her words as follows:
“Firstly, the underlying causes must be addressed. Considering that one of the main causes of tooth sensitivity is teeth clenching and bruxism, performing procedures aimed at treating bruxism can significantly help reduce sensitivity. Secondly, aggressive brushing should be avoided. To prevent this situation from worsening, it is important not to brush teeth with excessive pressure or harshly. Thirdly, the use of toothpastes containing sensitivity-reducing agents can be very effective. Additionally, brushing with toothpastes high in fluoride content can also be beneficial for alleviating sensitivity.

If the problem persists despite all these applications, professional support must be sought.”
