## Özet

**President of Üsküdar University, Psychiatrist Prof. Nevzat Tarhan, attended the 24th National Neuroscience Congress, organized by Boğaziçi University Institute of Biomedical Engineering with the contribution of the Brain Research Association, themed “Innovative Approaches from Neuroscience to Neurotechnology”. Tarhan gave a presentation titled “Computational Psychiatry and Mind Paradigm Dtx: Digital Therapeutics”. He drew attention to the transformation of digitalization and artificial intelligence in the field of health. Stating that the artificial intelligence-supported treatment approach is gaining importance in neuropsychiatry, Tarhan evaluated the developments in human brain connectivity and decision-making mechanisms, as well as brain imaging technologies.  **

The congress, held at Boğaziçi University South Campus Albert Long Hall, was attended by many experts in their fields as speakers, including President of Üsküdar University, Psychiatrist Prof. Nevzat Tarhan, Prof. Kemal Arıkan, faculty member of the Department of Medical Statistics, Üsküdar University Faculty of Medicine, and NPİSTANBUL Hastanesi Neurology Specialist Prof. Barış Metin, among others.

## **“The healthcare sector is one of the leading fields where digitalization is most widely used” **

President of Üsküdar University, Psychiatrist Prof. Nevzat Tarhan, gave a presentation titled “Computational Psychiatry and Mind Paradigm Dtx: Digital Therapeutics”. Tarhan emphasized that the impact of digitalization in healthcare is growing and digital health data is gaining importance; “Over the last decade, our digital health footprint has been steadily growing. We have entered an era where digital medicines, or ‘Digital Therapeutics,’ are being discussed. Now, like pharmacological drugs, digital drugs are also being seriously debated. The most important reasons for this are digitalization, electronic health records, and genome research. The healthcare sector is one of the leading fields where digitalization is most widely used globally because it greatly simplifies processes. Turkey became one of the most successful countries in this regard during the pandemic period, making a significant difference globally with its e-Nabız system. Currently, a large amount of data has been collected. Health data grows by approximately 40-45 percent each year. The healthcare sector accounts for approximately 30 percent of the total data produced worldwide. In 2013, then-US President Obama emphasized the importance of electronic health records, neurogenetics, and neurotechnology when he launched the BRAIN Initiative. Indeed, Clinton, who initiated the Genome Project in 2003, also stated in a 2013 presentation, ‘We did the genome project. We sold the technology we made for 1 dollar to the world for 147 dollars.’ Now, we see that a similar outcome is desired in brain research, using technology with the same goal,” he said.

## **“Artificial intelligence is rapidly entering our clinical practice as a decision support system”**

Tarhan pointed out that the concept of cognitive psychology in Turkey is often confused with Cognitive Behavioral Therapy and that artificial intelligence is increasingly being used in neuropsychiatry; “In Turkey, when cognitive psychology is mentioned, it is usually understood as Cognitive Behavioral Therapy (CBT). In fact, cognitive psychology is the branch of psychology that treats the brain like a computer. For example, at METU, it exists as a department of cognitive psychology. We also wanted to establish and open it, but unfortunately, we could not get this structure of cognitive psychology accepted. Ultimately, we can carry out many studies within the scope of neuroscience. While the fundamental question of the 1950s and 60s, when computers first emerged, was ‘Can machines learn?’, today's fundamental question has become ‘Can machines feel?’ Currently, the question ‘Can machines truly feel?’ is being debated in our minds. At this point, the era of artificial intelligence-supported treatment has begun in neuropsychiatry. Even though the specialist physician will still make the final decision, artificial intelligence is rapidly entering our clinical practice as a decision support system,” he stated.

## **“The human brain operates with fuzzy logic”**

Tarhan drew attention to the relationship between the human brain's working mechanism and artificial intelligence systems; “Today, we can see how seriously artificial intelligence affects our decision support systems. The most critical question in the principle of connectivity in the brain is ‘Is everything something, and is something everything?’ The human brain works with probability mathematics. Unlike Newtonian mathematics, which operates with super determinism or black-and-white (1-0) logic, the human brain functions with fuzzy logic. Hinton, while developing the deep learning system, introduced the Boltzmann machine model by calculating the gray areas between black and white. This provided the fundamental discoveries that made machine learning with artificial neural networks possible,” he said. 

## **A simple imitation of the brain’s decision-making mechanism…**

Tarhan stated that the complex structure of the human brain and its decision-making mechanism can be mathematically modeled; “Deep learning is not just about 1s and 0s, but also about the ability to calculate other intermediate tones. The human brain is a complex structure that can be defined not in 2 or 3, but in 7 dimensions. In addition to structural and functional connections, there are ‘click’ connections where small neuron groups connect and ‘cavity’ connections where larger groups connect. Obtaining this data makes brain functions measurable in computational psychiatry. The Blue Brain Project, initiated in Switzerland and using algebraic topology architecture, was also one of the first studies to prove the mathematical operation of the brain. This system creates an algebraic topological map by combining neural networks with neural genetic algorithms. With the modeling, spatial dimensions and the topological dimension of geometric objects can be defined. Thus, we can track the brain's activity pattern in response to a stimulus and the visual interpretation process. In all these processes, information is decoded, processed, and ultimately a decision is reached. Computers also make decisions using this algebraic topology architecture, which is essentially a simple imitation of the human brain's decision-making mechanism,” he said.

## **“Our brain functions like an organ of belief”**

Tarhan emphasized that the brain is a structure that operates not only with emotions and thoughts but also with beliefs and probability calculations; “Our brain functions not merely as an organ of emotion or thought, but rather as an ‘organ of belief’. The brain performs probability calculations and is drawn to probabilities, which we call rational belief. Reason operates with information and data. The more data a person has, the more accurate decisions they can make. Therefore, databases have become very important today. This bidirectional flow of information in cortical columns forms the basis of our decision system. According to the philosophy of mind, the human brain is much larger and more comprehensive than the body. At this point, quantum mechanics and quantum computation come into play. Whether our mind and brain function like a receiver resonating in the quantum field is still an important question. According to String Theory, there is no solid matter in the universe; everything consists of electronic threads of oscillation and vibration. What we call matter is actually a tangle of oscillations. It is unthinkable that this reality would not affect psychiatry, neuroscience, and the entire medical world. Currently, we apply network analyses in our clinical practice. We obtained this data from Stanford University's personalized precise psychiatry publications and evaluated it in our multidisciplinary meetings,” he stated.

## **The real-time functioning of the human brain can be mapped**

Tarhan emphasized the importance of evaluating diseases through neural networks and of functional brain imaging technologies in neuropsychiatry; “When we look at the neural networks in diseases, we find the ‘default mode network’ related to awareness or mindfulness, the ‘salience network’ affected by serotonin reuptake inhibitors, dopamine/noradrenaline networks related to negative and positive mood states, and attention and cognitive control networks. The mappings obtained regarding how the frontal and parietal lobes or the amygdala connect with which networks provide information about which area is a candidate network for that disease. However, the final decision must always be made by the clinician, by eliminating possible false positive/negative results. In the future of neuropsychiatry, we will no longer simply name diseases as ‘schizophrenia’ or ‘bipolar’. We will state which regions of the brain have a disconnection (disconnectivity) or ‘connectome’ disorder. Thanks to functional brain imaging technologies, the real-time functioning of the human brain can be mapped. Here, the biggest advantage of QEEG (Quantitative EEG) over other imaging techniques is that it offers instantaneous timing capability,” he said. 

## **“Humanity’s search for meaning has become more important than everything else”**

Tarhan drew attention to the relationship between neuroscience and the science of meaning; “To establish the relationship between neuroscience and the science of meaning, let's recall Maslow's Hierarchy of Needs. While Maslow initially placed self-actualization at the top of the pyramid, in a book he published before his death, he changed this apex to ‘self-transcendence,’ meaning the need to help others. The renowned psychiatrist Viktor Frankl also argued that even if basic needs are not met, humanity's search for meaning becomes more important than anything else. Viktor Frankl, as a 38-year-old physician who fell captive in Nazi camps, was deeply affected by the March 30th Incident he experienced. A friend of his dreamt that he would be saved on March 30th and believed in it. However, March 30th arrived, the dream did not come true, and his friend, who lost hope, died within a few days. Frankl personally observed that the meaning attached to holding onto life produced a biological and neurophysiological outcome, and the loss of hope led to physical death. Studies on neuroplasticity developed with focus and motivation also confirm this,” he stated.

## **Prof. Kemal Arıkan spoke about the importance of preventive medicine**

Prof. Kemal Arıkan, faculty member of the Department of Medical Statistics, Üsküdar University Faculty of Medicine, gave a presentation at the 24th National Neuroscience Congress titled “Preventive Medicine in Psychiatry: The Role of QEEG in Predicting Depression Recurrence”. 

Prof. Kemal Arıkan, in his presentation, discussed the use of QEEG (quantitative EEG) in predicting the risk of depression recurrence. He drew attention to the contributions that QEEG data could provide to predicting depression recurrence and preventive medicine practices.

## **Prof. Barış Metin explained the contributions that artificial intelligence can provide to diagnostic processes in neuropsychiatry**

NPİSTANBUL Hastanesi Neurology Specialist Prof. Barış Metin also attended the 24th National Neuroscience Congress. Metin gave a speech titled “Can artificial intelligence provide reliable diagnostic support tools in neuropsychiatry?” 

Prof. Barış Metin, in his speech, discussed the contributions that artificial intelligence can provide to diagnostic processes in neuropsychiatry. He drew attention to the fact that artificial intelligence can support clinical decision-making processes and the importance of the reliability of these technologies.
