## Özet

**Üsküdar Üniversitesi Founding Rector, Psychiatrist Prof. Dr. Nevzat Tarhan, was a guest on the “Alişan ile Gülümse Hayata” (Alişan and Smile to Life) program aired on TRT 1 to evaluate the psychological impact of the earthquake that affected 10 provinces and plunged all of Turkey into mourning. Tarhan, who mentioned that psychological needs will emerge after basic needs are met, said: “If normalization is not achieved, trauma prolongs, and when trauma prolongs, the feeling of hope weakens.”**

Üsküdar Üniversitesi Founding Rector – Psychiatrist Prof. Dr. Nevzat Tarhan, who participated in the Alişan ile Gülümse Hayata program aired on TRT 1, made striking statements about earthquake trauma and acute stress disorder.

## **It is necessary to make individuals feel that you are with them…**

Üsküdar Üniversitesi Founding Rector Tarhan underlined that if the shock period lasts longer than three days, professional help is required. Tarhan stated: “In the initial stages of shock, earthquake victims cannot feel much. The person does things unconsciously at that moment. This sometimes lasts a day, sometimes three days, sometimes a little longer. This is a state of bewilderment. They wander aimlessly or don’t remember what they did later. A person has such a period, they experience the trauma firsthand. This is referred to in the literature as acute stress disorder. This condition exists, but then it improves within a few days. People start to recover afterward. Meanwhile, those who arrive from the surroundings are more helpful. It is generally expected to recover within three days. If it exceeds three days, expert help is needed. Some people, at that moment, attack left and right. They exhibit such angry behaviors, causing injuries. There are people here who injured rescue teams. Someone showed me how they bled their leg; the person they were trying to rescue had lost control. Such situations will occur. In the past, what we called briefing—making sense of things—was done. For example, taking them to therapy individually is not done now because it was found to be problematic. That was the method developed by the Israelis. The current method is to make these people feel safe and that you are with them. There is no need to talk extensively next to them. It is enough for them to feel that their basic needs are met; when the person grasps the feeling, 'I am safe, I am being helped here in an organized way, institutions related to me are helping me here,' their anxiety and panic immediately subside.”

## **Sister cities can be declared**

Prof. Dr. Nevzat Tarhan stated that delays could be minimized if sister cities were declared. Tarhan said: “There were transportation difficulties. For example, one of the first aid providers who arrived the next day was Ordu Municipality; they had probably come to Kahramanmaraş. It was the very first day, and they saw it immediately. Similarly, if Istanbul, for instance, were declared a sister city to Izmir, Erzurum, Samsun, Adana, in such a situation, if something like this happens automatically, there would be no need for instructions. It would automatically send its ready system there immediately. So, let this be one of the lessons we learn from this. By declaring such sister cities, they would complement each other like 'Buddies'. If this happens, we would minimize the delays of the first day. In such situations, for first aid, for those left stranded, because the first 24-48 hours are very critical.”

## **Trauma Lengthens if Normalization is Not Achieved!**

Prof. Dr. Nevzat Tarhan made important evaluations regarding the fact that basic needs are currently suppressing psychological needs: “The issue of psychological help will generally emerge more prominently after two or three weeks. Currently, basic needs such as shelter, food, and drink are very important. After these are met, psychological needs start to come to the forefront. If necessary, in the form of online treatments… For example, as a team, we are preparing short videos on this; 'What is first psychological aid, what is secondary trauma, what to do in acute stress disorder, what is post-traumatic growth?' We will try to prepare and share dozens of videos on all these topics. People can listen to these and extract insights for themselves, in a psychoeducation style; we are trying to do these. For psychological support, unless there is an acute reaction, meeting the basic needs of those people is the greatest support. Basic needs are currently suppressing psychological needs. Psychological needs emerge after basic needs are met, but they appear more prominently in those with pre-existing conditions or those who are vulnerable. Generally, there is a 4-week period here; it is necessary to transition to normalization between 7-14 days; currently, it is beneficial for us to transition within 1 week. If normalization is not achieved, trauma prolongs, and when trauma prolongs, the feeling of hope weakens. There are crimes committed when hope runs out, public order is disrupted. Therefore, it is necessary to establish order and transition to normalization within 7-14 days, otherwise, the problem becomes chronic, and when the problem becomes chronic, people start to feel anxiety about the future, cannot tolerate it, and negative propaganda becomes more effective.”

## **It is extremely important to endure with tolerance…**

Tarhan, who stated that in such situations, instead of complaining, when people realize the value of what they have and endure them, windows open before them: “Earthquake victims experienced the pain of this trauma; this is a very great test for them. In such tests, it is very important to be able to endure with tolerance. The second test here is for those who experience the earthquake secondarily. From afar, we are not in the earthquake, but their test, which we call secondary trauma in a similar way to the earthquake, is the test of being able to help the victims and the oppressed there. If both sides, the victims in the test, succeed in waiting with resilience, tolerance, perseverance, and patience without falling into despair—and patience is not about withdrawing and waiting. Patience is about adapting to the speed and rhythm of nature. Patience is a meditative act. It relaxes a person; patience is bitter, but its fruit is sweet. Its benefits emerge later. Therefore, in such situations, instead of complaining, when you realize the value of what you have and appreciate what you currently possess and endure, then the windows before us open. The developments behind this calamity emerge afterward. In such situations, instead of complaining to a person with a broken arm, 'Why is your arm broken?', it is like acting with 'What is the remedy in such situations?' And the second test is even more important. To understand those people, in our Anatolia, it is called 'Halden anlamak' (understanding one's situation). Empati, diğerkâmlık (empathy, altruism) the feeling of empathy is still alive in us.”
