## Özet

Experts say that treatment adherence in bipolar disorder and schizophrenia cannot be explained solely by the patient's desire to take their medication regularly.

## İçerik

Psychiatry Specialist Asst. Prof. Elvan Çiftçi, stating that disease awareness, beliefs about medication, social support, and the doctor-patient relationship play an important role in treatment adherence, said, 'The more information one has about the disease, the more committed one remains to the treatment; moreover, information, adherence, and therapeutic alliance change together over time.' Asst. Prof. Çiftçi emphasized that family support, when correct and collaborative, strengthens treatment adherence, while an overly critical attitude can have the opposite effect, and noted that long-acting injections can increase the continuity of medication use, but these also require patient acceptance and regular follow-up.

Asst. Prof. Elvan Çiftçi, Psychiatry Specialist at Üsküdar University NPİSTANBUL Hastanesi, made statements about the factors affecting treatment adherence in bipolar disorder and schizophrenia and the role of patient awareness, family support, therapeutic relationship, and long-acting drug treatments in increasing adherence.

**Treatment adherence is not a willpower issue, but a clinical problem that needs to be understood!**

Asst. Prof. Elvan Çiftçi stated that for many patients diagnosed with bipolar disorder or schizophrenia, the biggest challenge is not the diagnosis itself, but being able to continue treatment for a long time. She said, 'Research shows that treatment adherence is a multi-factorial and complex problem. In this context, the prominent basic elements are; illness insight, attitudes and beliefs towards medication, substance use, social support, and the quality of the therapeutic relationship.'

In other words, Asst. Prof. Çiftçi stated that whether a person regularly takes their medication is rarely a matter of 'wanting or not wanting,' but rather a matter of 'understanding or not understanding,' 'believing or not believing,' and 'receiving or not receiving support.'

**Providing information to the patient is not enough for treatment adherence!**

Pointing out that the most reliable way to increase treatment adherence in bipolar disorder is not simply lecturing the patient, Asst. Prof. Çiftçi said, 'Interventions that help the patient truly understand their illness over time, change their beliefs about medication, and develop self-management skills are more valuable.'

**Asst. Prof. Çiftçi gave examples from studies and said the following:**

'In an outpatient follow-up study, the patient's knowledge about treatment, mental health status, and awareness of psychotic experiences showed a positive relationship with adherence both at baseline and at one-year follow-up. A one-year observational study covering both bipolar disorder and schizophrenia patients yielded a similar result: The more information one has about the disease, the more committed one remains to the treatment; moreover, information, adherence, and therapeutic alliance change together over time.

When it comes to schizophrenia, the picture is more complex. Research evidence shows that no single intervention has a consistent and strong effect. What is truly effective is a personalized and multi-component approach; that is, usually a combination of behavioral interventions, education, family support, and pharmacological strategies.'

**Being aware of the disease increases adherence to treatment!**
 />

Asst. Prof. Çiftçi stated that insight, meaning a person's awareness of their own illness, increases medication adherence in both disorders, adding, 'However, this connection has been shown to be stronger and more consistent in bipolar disorder.'

Noting that the relationship between insight and adherence in schizophrenia is also clear, Asst. Prof. Çiftçi said, 'However, this relationship is not one-dimensional. Cognitive insight and clinical insight contribute to adherence together; in schizophrenia, cognitive insight plays a complementary role to clinical insight. A one-year observational study also suggested that insight can indirectly affect adherence through the therapeutic alliance, meaning that patients with good insight can establish a better relationship with their doctors, which increases their commitment to treatment.'

**Family support is a strength if done correctly, a burden if done incorrectly!**

Asst. Prof. Çiftçi stated that family support generally increases adherence to schizophrenia treatment, adding, 'However, the condition for this is that the support must be practical, continuous, and collaborative.'

Asst. Prof. Çiftçi emphasized that when the family provides structure, monitoring, encouragement, and informed help through psychoeducation or multi-family approaches, it best facilitates treatment adherence, and continued:

'However, evidence also shows that supportive involvement does not always and automatically yield benefits. When families are overburdened, overly critical, or lack sufficient information, adherence can worsen instead of improve.

Therefore, family psychoeducation should be considered an indirectly working supportive intervention. It helps the patient by increasing understanding, support, and participation. The most significant finding is that structured interventions involving the family, especially programs tailored to beliefs about medication use and family involvement, tend to yield better results than education solely for patients or usual care practices.'

**Long-acting injections are advantageous but not magical!**

Referring to worldwide studies showing that long-acting injectable antipsychotics provide higher medication adherence and lower treatment discontinuation rates compared to oral antipsychotics in the treatment of schizophrenia, Asst. Prof. Elvan Çiftçi said, 'The biggest advantage of long-acting injectable antipsychotics is the certainty of drug administration. The drug is administered throughout the dosage interval; thus, the risk of missing daily doses decreases, and the detection of non-adherence to medication use becomes easier.'

However, Asst. Prof. Çiftçi underlined that success does not come automatically, and concluded her words as follows:

'Patient acceptance is important. Satisfaction, preference alignment, and belief in preventing relapse increase the acceptance of long-acting injections. There is a balance. Adherence no longer depends on daily self-medication behavior but on participation in injection and follow-up appointments, which means that missed appointments become a new point of weakness.

In some early psychosis studies, even if an improvement in relapse or hospitalization rates was observed, no significant difference in adherence was found between long-acting injections and oral medications.'
