## Özet

**Üsküdar University and NPİSTANBUL Hastanesi participated in the SBMT 23rd Annual Neurotechnology Congress held in the USA with a strong academic staff, as part of the global neurotechnology consortium they have been a part of for over 10 years. They delivered presentations covering a wide range of topics, from anhedonia to artificial intelligence models, EEG biomarkers to addiction treatments. **

**Psychiatrist Prof. Nevzat Tarhan emphasized that anhedonia, defined as an individual's inability to experience pleasure from activities normally enjoyed, consists of different components such as lack of motivation, inability to feel pleasure, and inability to learn from reward.**

**Prof. Nesrin Dilbaz stated that Transcranial Magnetic Stimulation (TMS) applications, used in the treatment of mental health issues like depression and anxiety, also offer promising results in the fight against addiction.**

**Prof. Türker Tekin Ergüzel discussed the development of fuzzy logic techniques in healthcare and their integration with modern artificial intelligence systems, emphasizing that these techniques are expected to be used more widely in personalized medicine and real-time clinical decision support systems in the near future. **

**Prof. Barış Metin drew attention to the importance of using electroencephalography (EEG), and particularly quantitative EEG (qEEG), in the treatment of depression.**

**Asst. Prof. Alptekin Çetin emphasized that anhedonia, despite being one of the core symptoms of depression, is often not adequately targeted.**

## **Digital Transformation in Mental Health: AI and Brain Mapping Converge**

Üsküdar University and NPİSTANBUL Hastanesi participated in the congress with a strong academic staff, as part of the global neurotechnology consortium they have been a part of for over 10 years. The delegation was led by President of Üsküdar University and Psychiatrist Prof. Nevzat Tarhan; Prof. Nesrin Dilbaz, Prof. Türker Ergüzel, Prof. Barış Metin, Dr. Alptekin Çetin, and NPİSTANBUL Hastanesi Board Member Fırat Tarhan contributed with presentations on anhedonia in depression and addiction, the relationship of anhedonia with economic performance, electrophysiological biomarkers, and transformative fuzzy logic-based AI models in two separate sessions.

## **Multidimensional approach to anhedonia**

In his presentation titled “Neurogenetic Bases and Treatment Approaches of Anhedonia”, Prof. Nevzat Tarhan addressed anhedonia as a multidimensional neuropsychiatric condition, emphasizing that anhedonia, defined as an individual's inability to experience pleasure from activities normally enjoyed, consists of different components such as lack of motivation, inability to feel pleasure, and inability to learn from reward.

Prof. Tarhan, who focused particularly on the brain's mesolimbic dopamine system in his presentation, stated that a communication disorder between the ventral striatum and the prefrontal cortex underlies this condition. Prof. Tarhan also noted that inflammation deepens anhedonia by suppressing dopamine production, and neuroinflammation is a significant risk factor.

## **“Serotonin-focused treatments are often insufficient”**

Prof. Tarhan, also drawing attention to the genetic basis of anhedonia, stated that dopamine receptors, BDNF, and stress-related genes play a role in susceptibility to the condition. Prof. Tarhan expressed that from a clinical perspective, serotonin-focused treatments are often insufficient, and dopaminergic and glutamatergic approaches might be more effective.

Prof. Tarhan stated that methods such as behavioral activation and positive psychotherapy could make significant contributions to treatment, adding that the brain's restructuring might be possible through the neuropsychological foundations of hope.

## **Addiction and anhedonia fuel each other**

Prof. Nesrin Dilbaz, Faculty Member at Üsküdar University and Psychiatrist at NPİSTANBUL Hastanesi, addressed the relationship between anhedonia and addiction in her presentation titled “The Relationship Between Anhedonia and Addiction”, emphasizing that impairments in the reward system not only reduce the capacity to experience pleasure but also make individuals more prone to addictive behaviors. 

Prof. Dilbaz stated that as dopamine-based reward mechanisms weaken, individuals cannot derive sufficient satisfaction from natural rewards and tend to fill this void with substance use or addictive behaviors.

Prof. Dilbaz, drawing attention to the fact that addiction and anhedonia are two mutually reinforcing processes, noted that chronic substance use further impairs the reward system, deepening anhedonia. Prof. Dilbaz emphasized that, therefore, treatment approaches should target not only the addictive behavior but also the underlying anhedonic structure.

Prof. Dilbaz stated that studies conducted within research centers have shown that addressing these two conditions together increases treatment success.

## **Deep TMS treatment in alcohol and substance addiction**

In her second presentation, Prof. Nesrin Dilbaz stated that Transcranial Magnetic Stimulation (TMS) applications, used in the treatment of mental health issues like depression and anxiety, also offer promising results in the fight against addiction.

Prof. Dilbaz stated that Deep TMS, in particular, was developed to provide a more powerful and targeted treatment by acting on deeper brain regions, emphasizing that this method has significant potential in alcohol and substance addiction, as well as other types of behavioral addictions.

Prof. Dilbaz stated that the effects of Deep TMS treatment on addiction are becoming clearer with recent clinical research, and that this method, which directly affects the brain's reward system, can provide successful results with fewer side effects compared to traditional treatments.

Prof. Dilbaz added that Deep TMS offers a safe alternative, especially for treatment-resistant individuals, and creates new hope in the fight against addiction.

## **Artificial intelligence and fuzzy logic open new doors in healthcare**

Prof. Türker Tekin Ergüzel, Vice Rector of Üsküdar University, discussed the development of fuzzy logic techniques in healthcare and their integration with modern artificial intelligence systems in his presentation titled “Hybrid Fuzzy Logic and Transformative Model Applications in Healthcare”.

Prof. Ergüzel stated that fuzzy logic provides significant advantages in health data due to its ability to analyze uncertain and ambiguous data, going beyond the classical true-false approach. He noted that these methods, widely used in medical imaging and biosignal analyses such as MRI, CT, and EEG, achieve high accuracy rates in diagnosis and clinical decision support processes.

## **Real-time clinical decision support systems will be more widely used in the near future**

Prof. Ergüzel stated that in recent years, fuzzy logic has combined with machine learning and deep learning techniques to create hybrid models, allowing for both performance and interpretability. He shared that they use these hybrid systems under the title “NP Model” and have achieved very effective results.

In the presentation, Prof. Ergüzel also drew attention to some limitations of fuzzy logic-based systems, in addition to their advantages, explaining that issues such as model complexity, computational costs, and generalizability are still areas that need further development.

Prof. Ergüzel emphasized that these techniques are expected to be used more widely in areas such as personalized medicine, real-time clinical decision support systems, and multi-data integration in the near future. He highlighted that research has shown that through the integration of fuzzy logic with deep learning, more reliable and explainable artificial intelligence solutions can be developed in healthcare, once again demonstrating the importance of hybrid models, especially in the field of neurotechnology.

## **EEG biomarkers guide treatment**

Prof. Barış Metin, Faculty Member at Üsküdar University and Neurologist at NPİSTANBUL Hastanesi, also highlighted the importance of using electroencephalography (EEG) and especially quantitative EEG (qEEG) in the treatment of depression in his presentation, emphasizing that the identification of electrophysiological biomarkers has become a crucial research area for diagnosis, treatment planning, and prognosis prediction.

Prof. Metin stated that EEG has high applicability in clinical practice due to being a non-invasive, low-cost, and repeatable method, adding that biomarkers can be used to predict treatment resistance, select appropriate pharmacotherapy, and identify high-risk individuals.

Prof. Metin shared examples from studies conducted at NPİSTANBUL Hastanesi, stating that deep learning-based analysis approaches can classify antidepressant responses with high accuracy rates using pre-treatment EEG data. Prof. Metin noted that significant results were obtained particularly in predicting Sertraline, Bupropion, and placebo responses.

Prof. Metin also drew attention to the possibility that high gamma activity in EEG may be related to suicidal behavior and that increased power values in specific electrode regions could be considered a potential biomarker. 

At the end of his presentation, Prof. Metin emphasized the need for longitudinal EEG studies to determine treatment discontinuation time, predict relapse risk, and identify resistant cases.

## **Necessity of anhedonia-focused treatment in depression**

Asst. Prof. Alptekin Çetin, Psychiatrist at Üsküdar University NPİSTANBUL Hastanesi, emphasized in his presentation titled “Anhedonia in Depression” that anhedonia, despite being one of the core symptoms of depression, is often not adequately targeted.

Çetin stated that anhedonia is not merely a loss of pleasure; it is also a multidimensional construct characterized by a decrease in motivation, interest, and reward anticipation. He reported that research shows anhedonia is not unique to depression but is a "transdiagnostic" symptom appearing in many conditions such as schizophrenia, addiction, and various neurological diseases.

Dr. Çetin also noted that anhedonia has a strong relationship with loss of functionality, treatment resistance, relapse risk, and suicidal tendencies. In his presentation, he included findings on the brain's reward system, particularly drawing attention to the role of dopamine in motivation and reward learning processes. Dr. Çetin stated that neuroimaging studies show reduced ventral striatum activity and weakened connections with the prefrontal cortex in individuals with anhedonia. 

Dr. Çetin stated that classic antidepressants are limited in some cases for treatment, expressing that dopaminergic agents, fast-acting treatments like ketamine, and neuromodulation methods hold promise. He added that personalized treatment approaches and brain mapping techniques will play a more significant role in the future.

## **Mental health also affects the economy**

Fırat Tarhan, Member of the Board of Directors of NPİSTANBUL Hastanesi, addressed mental health as one of the unseen determinants of the global economy in his presentation titled “From Dopamine to GDP: The Relationship of Anhedonia with Economic Performance”.

In the study titled “From Dopamine to GDP”, Fırat Tarhan emphasized that the dopamine system, which governs individuals' motivation, yields not only psychological but also economic consequences, stating, “Anhedonia, that is, the loss of pleasure and motivation, directly reduces productivity by weakening individuals' goal-oriented behaviors.”

Fırat Tarhan stated that in the research, the loss particularly in the "wanting" (motivation) dimension reduces individuals' effort and participation in economic activities, leading to a micro-level loss of productivity. He noted that these individual effects eventually transform into macroeconomic consequences, and anhedonia constitutes a significant productivity cost on a global scale.

Fırat Tarhan, drawing attention to the loss of billions of workdays worldwide and an economic loss of approximately 1 trillion dollars largely due to lack of motivation, stated that anhedonia also has negative effects on consumption, innovation, and the labor market. Tarhan emphasized that, in this context, individuals take fewer risks, consume less, and invest less in long-term goals.

Fırat Tarhan stated that mental health should be considered a fundamental policy area not only for individuals but also for economic growth and development, concluding his presentation with current statistics and numerical data illustrating the relationship of anhedonia with economic performance.
