Experts state that long-term hunger and thirst can affect the electrical balance of the brain in epilepsy patients, adding that this situation can increase the risk of seizures.

Neurology Specialist Dr. Celal Şalçini drew attention to the fact that shifting medication times according to suhoor and iftar can lead to a decrease in drug levels in the blood and the recurrence of seizures that were previously under control. He stated, “During the period without medication, the brain’s protective shield weakens, and even a single missed or delayed dose can disrupt a seizure-free period lasting months, or even cause severe seizures requiring hospitalization.” Emphasizing that patients with resistant epilepsy and those using multiple medications should be careful about fasting, Dr. Şalçini warned that even if seizures are under control, the decision to fast must be made after a doctor’s evaluation. 

Üsküdar Üniversitesi NPİSTANBUL Hastanesi Neurology Specialist Dr. Celal Şalçini provided information about the risks epilepsy patients may face when fasting during Ramadan.

**Long-term hunger and thirst can affect brain function!**

Dr. Celal Şalçini stated that long-term hunger and thirst can affect the brain’s working order by altering our body’s delicate balance. He said, “Especially a drop in blood sugar (hypoglycemia) and dehydration of the body can disrupt the electrical balance of nerve cells, lowering the protective limit we call the ‘seizure threshold.’”

Dr. Şalçini emphasized that this situation can make the brain more susceptible to electrical storms and trigger unexpected seizures in a patient who normally does not experience seizures or whose seizures are under control.  

**Shifting medication times can increase seizure risk!**

Dr. Celal Şalçini stated that the most critical rule in epilepsy treatment is to keep drug levels in the blood constant. He said, “Shifting medication times according to suhoor and iftar (for example, taking medication that should be taken every 12 hours with gaps of 16-17 hours) can lead to drug levels in the blood dropping to dangerous levels. During this ‘medication-free’ period, the brain’s protective shield weakens, and even a single missed or delayed dose can disrupt a seizure-free period lasting months, or even cause severe seizures requiring hospitalization.”

**Not every epilepsy patient is suitable for fasting!**

Dr. Celal Şalçini pointed out that sleep is a vital component of treatment for epilepsy patients, at least as much as medication. He said, “Waking up for suhoor, going to bed late, or waking up early in the morning creates ‘sleep deprivation,’ causing brain cells to become hypersensitive.”

Underlining that insomnia is the strongest factor triggering seizures in many types of epilepsy, Dr. Şalçini continued:

“Therefore, the irregular sleep routine during Ramadan carries a serious risk of increased seizure frequency. Not every epilepsy patient is suitable for fasting. Especially for patients with ‘resistant epilepsy’ whose seizures continue despite medication, those who have to use more than one or more than three medications a day, those whose seizures are triggered by insomnia, and patients in childhood/old age, fasting is not recommended. Furthermore, even if seizures are under control, it is critically important for individuals with a history of severe, life-threatening seizures not to make this decision without consulting their doctors.”

**Fasting should be stopped and a doctor consulted at the slightest sign of a seizure!**

Dr. Celal Şalçini concluded by stating that if seizures have been under complete control for a long time, only one type of medication is being used, and there is doctor’s approval, safe fasting may be possible. He warned, “During this process, plenty of fluids should be consumed between iftar and suhoor, medications should be taken strictly according to the new time frames recommended by the doctor, and most importantly, rest periods should be created to compensate for sleep loss during the day. However, if even the slightest seizure or seizure warning (aura) is felt while fasting, fasting should be stopped immediately to avoid risking health, and the situation should be reported to the doctor.”
